What are the responsibilities and job description for the FT MDS Coordinator position at PioneerCare?
The MDS Coordinator is a RN who coordinates and directs the nursing activities at PioneerCare and works with adult and geriatric residents. The RN MDS Coordinator is responsible for the professional assessment and coordination of care needed for the residents, nursing staff and overall operation of the facility to ensure a high standard of quality care. Coordinates care provided by other members of the health care team, makes referrals for follow-up care and consults with physicians and others to provide individualized care. The RN MDS Coordinator supports and assures the policies and procedures of PioneerCare are adhered to and is responsible and reportable to the Director of Nursing in coordinating the total care of residents.
This is a Full TIme Registered Nurse position. The hours are Monday - Friday 8 a.m. to 4:30 p.m.
