Job Post: RN/LPN – Wellness Clinic
Piper Shores is Maine’s premier nonprofit continuing care retirement community, offering Independent Living, Assisted Living, and Skilled Nursing Care with a person-centered approach to care.
We are looking for an RN/LPN to oversee operations of the Wellness Clinic, serving Independent Living Residents through restorative nursing, fitness, and promoting independence and well-being.
This is a full-time, Monday-Friday position with hours from 8:30am to 5 pm
Responsibilities:
· Foster a care environment that promotes dignity, autonomy, and individualized treatment plans.
· Monitor and assess resident conditions, responding promptly to changes in health status and providing direct care as needed.
· Ensure care plans are collaboratively developed with residents and their families, reflecting their goals and priorities.
· Coordinates and oversees the restorative nursing program and acts as a liaison between therapy and nursing service; ensures that restorative nursing programs attain goals of assisting residents to achieve or maintain their highest level of function.
· Oversees contracted fitness program through UNE.
· Supervise and mentor one Wellness CNA, overseeing scheduling, training, and performance.
· Evaluates resident safety and coordinating for interventions – collaborating with interdisciplinary team to coordinate care either in independent living or a higher level of care.
· Coordinates in-home care services through the wellness center or outside providers.
· Maintain compliance with regulatory requirements for Home Health and Personal care agency licensing
· Maintains appropriate and complete reports, records, physician’s orders, service plans, and progress notes as needed.
· Offers or coordinates emotional support to residents and family members within the aging process and/or illness.
· Assists in coordinating services for residents being admitted to and discharged from the Health Center.
· Participates in rotating on-call coverage
Qualifications and Work Environment
- Actively licensed RN or LPN by the state of Maine
- 3-5 years working in geriatrics in long-term care, home health or other health care setting with prior supervisory experience preferred.
- Warm and friendly demeanor with a positive approach to interacting with Seniors and co-workers.
- Strong communication skills, both written and verbal.
- PC knowledge and skills, including MS Word and EMR systems.
- Current CPR certification or ability to obtain within 30 days of employment.
