What are the responsibilities and job description for the ECM Case Manager position at Pit River Health Service, Inc?
The ECM Case Manager’s primary focus is assisting providers and clients in the areas of serious mental illness and/or substance use disorder needs, adults living in the community and at risk for long-term care institutionalization, and children/youth involved in, child welfare (including foster care up to age 26) and rehabilitation aftercare. The ECM Case Manager reports directly to the BH Director.
Specific duties and responsibilities:
1. Work as part of the Behavioral health team to provide a full range of services with a primary focus on case management for clients in the areas of serious mental illness and/or substance use disorder needs, adults living in the community and at risk for long-term care institutionalization, and children/youth involved in, child welfare (including foster care up to age 26) and rehabilitation aftercare.
2. Work closely with the Provider(s) and clinical team in providing Case Management which may include:
a) Coordination of care within primary Care Team(s)
b) Assistance in management of patients with chronic illness with poor clinical outcomes or with complex healthcare/social needs
c) Assist patients in accessing their needs in the community.
d) Assist with the care transition for patients discharged from an inpatient setting.
e) Active participation in the recovery process for each client.
f) Provide home visits if needed.
3. Work with patients and Providers to assess, identify problems, plan goals, monitor and evaluate client care plans and develop strategies that meet the client’s immediate and long-term needs.
4. Assist patients with transportation to self help groups local and out of town.
5. Participate in patient and staff education at the direction of the BH Director. Work closely with clinical team to ensure compliance of the client’s treatment goals.
6. Coordinate tracking of patients for follow-up and continuity of care which may include:
a) Tracking outcomes and reporting to treatment team.
b) Tracking progress in the electronic health records system.
7. Assists providers with paperwork and collection of records needed to ensure quality patient visits.
8. Participates in the Case Management meetings with the Behavioral Health team and relevant departments.
9. Maintains patient confidentiality at all times; abides by HIPPA and other regulations put forth by the various regulatory agencies.
10. Performs other duties as assigned by the supervisor related with the general ambit of Case Manager.
Required Qualifications:
1. Must have a high school diploma.
2. Three years’ experience in case management or three years’ experience in related field.
3. Experience requirement can be waived if holding a bachelor’s degree in related field.
4. Must have Registered alcohol drug technician certification (RADT) within 90 days
5. Knowledge of community resources available.
6. Experience working with families and individuals who have mental illness and substance use disorder diagnosis.
7. Clean driving record and valid driver’s license. Willing to travel.
8. Effective communication skills.
9. Excellent organizational and time management skills.
10. Problem solving skills and ability to multi-task.
11. Must be able to work in a teamwork setting as well as demonstrate ability to work independently.
12. Must have great customer service skills and ability to communicate effectively and demonstrate cultural competency within the Native American community.
13. Maintain a current CPR card
14. Be willing to submit to and pass a pre-employment drug screening test.
15. Preference will be given to Native Americans in accordance with P.L. 93-638.

Pay: From $21.54 per hour
