Your Role:
- Manage and oversee patient care plans
- Collaborate with healthcare teams
- Educate patients and families
- Perform assessments & adjust care plans
- Ensure accurate documentation
- Active RN license
- 1 year of clinical nursing experience (home health preferred)
- Strong assessment & communication skills
- Ability to manage a caseload independently
