- Completes a comprehensive evaluation using appropriate evaluation and assessment tools and procedures
- Reviews patient records for current diagnoses, past medical history, precautions and contraindication
- Establishes measurable goals and develops treatment plan to move the patient toward his or her maximum level of independence and/or function
- Supervises physical therapist assistants and rehab techs in direct patient care and patient-related activities in accordance with state practice act.
- Interprets and communicates evaluation findings and treatment program to patients, family, and other team members
- Graduate from an accredited School of Physical Therapy
- Current licensure as a physical therapist in the state of current practice
- Must have good communication skills and be able to relate professionally and positively to patients, patients’ family members, and staff
- Must be capable of performing the Responsibilities of this job, with or without reasonable accommodations.
