Children's Case Manager

This clinical position within Pivotal’s Children’s Unit provides community-based case management services to severely emotionally disturbed children and adolescents presenting with moderate to severe levels of functional impairment (i.e CAFAS scores of 80-140 or MichiCANS equivalent). Although this position will take the lead role in providing case management services to the clients, it serves a team approach with other workers in the agency (as well as other agencies) that will often be utilized. This position works closely with Pivotal outpatient staff, Pivotal psychiatric staff, Juvenile and Family Court personnel, Children’s Protective Services, school and ISD personnel, and other private and public sector human service providers. This position provides a range of child- and family-focused clinical services and is generally performed on an outreach basis. This position may require the performance of additional duties as assigned. The case manager position allows for a more comprehensive approach in that it treats the child within the context of family and community parameters. It allows for more intensive intervention, and it enhances both child and family functioning, allowing greater ability to mobilize natural supports.

Pay: $16.79 - $27.00 per hour
