Placement Partners MN, Inc. was awarded a CADI/BI case management contract in Hennepin and now most recently, Ramsey County. We are currently seeking full-time Case Managers to meet the ongoing demand for case management services. You will work closely with the Case Management Supervisor and other community service providers to ensure that services are rendered to the persons served. Job Summary:
Provides the consumer and his/her family with access to assessments, Person-Centered Planning, referrals, linkage, support plan monitoring, coordination, and advocacy related to waiver services, resources, and informal supports that are not necessarily funded through the waiver.
Minimum Qualifications:
- Bachelor's Degree with an emphasis in Social Services, psychology, human services or other fields related to the education or treatment of people with developmental disabilities or related conditions.
- 1-2 years previous Waiver experience required.
- License in Social Worker preferred, but not required.
- Previous 1-year work experience providing case management services to persons with developmental disabilities is preferred, but not required.
- Valid Driver's License and Up to date auto insurance.
- Maintain acceptable motor vehicle records.
- Must be able to pass DHS Background Check

Job Type: Full-time
Pay: $23.00 - $27.50 per hour
