Job Type: Full-Time RN Home Hospice Case Manager
Schedule: M-F 8:00 AM - 4:30 PM, no weekends, backup on-call approx. 4 times/month
Sign-On Bonus: $10k with 2-year commitment
Responsibilities include coordinating patient care, assessing needs, and providing hands-on care in a home environment. Ideal candidates should have strong communication, analytical, and negotiation skills, with experience in pre-acute and post-acute care preferred.
Requirements: Associates Degree in Nursing (Bachelors preferred), current WI RN license, valid drivers license, BLS certification, and clinical experience preferred.
