About our Centralized Follow-Up Clinician position

Under the supervision of the Clinician Manager for Centralized Follow-Up [CFU] and the Chief Medical Officer, provides post-visit care to patients seeking reproductive health care in Health Centers in all affiliate states. Supports patients and staff by performing, with accuracy and timeliness, a variety of CFU tasks and incorporates those functions into the overall operations of the affiliate.
What you will be doing!
Providing direct and indirect patient medical care that is evidence-based and aligns with affiliate protocols. Performing tasks within the relevant scope of practice, including but not limited to triage, case management, follow-up on referrals and abnormal laboratory results, patient education, medical records review, and chart review

Maintaining knowledge of the affiliate Medical Standards and Guidelines. Maintaining ongoing competency and continuing education required for licensure and current knowledge needed to perform work tasks.

Exhibiting ongoing proficiency in electronic practice management systems/EHR (Epic), health center policies, procedures, and practices. Providing medical care in the electronic medical record environment. Documenting in medical record and practice management systems. Schedules appointments as needed.

Working within a team environment to render highly professional and technical medical care to patients. Assisting Clinical Services leadership in developing policies and procedures that promote teamwork, quality medical care, and enhanced customer service

Acting as a role model and resource in the development of follow-up systems. Managing communication with patients, colleagues, and persons external to the affiliate in a professional, systematic, and organized manner. Providing quality and efficient telecommunication or electronic customer service to internal and external customers.

Assisting in quality assurance tasks via auditing lab results, referrals, and follow-up plans to ensure completion of follow-up and adherence to established protocols based on affiliate Medical Standards and Guidelines. Completes quality assurance monitoring tools as specified for this department.

Must be willing to orient in surgical/family planning environment to develop in-depth knowledge of health center flow and functionality. Must be willing to orient in remote systems to develop in-depth knowledge of CFU processes.

Key Responsibilities:
The APC is assigned daily to a Centralized Follow-Up role and assumes the duties and responsibilities for that role for that shift; may offer support to other roles as time permits. The following are the major responsibilities across all Centralized Follow-Up Roles that may be needed as part of the CFU APC's responsibilities.
Providing professional and comprehensive medical assessment, evaluation, education, and advice to patients within affiliate protocols. Delegate work to Registered Nurses [RNs] within the scope of that licensure. Assisting and serves as consult resources to CFU/Triage RNs for aspects of patient care that are beyond the RN scope of practice.

Managing follow-up activities for abnormal results or physical findings. Activities may include maintenance of follow-up systems, notification of patients of results, scheduling of appointments, coordination of referrals to appropriate health care resources, and documentation of compliance with recommended medical regimens.

Providing patient education regarding topics within the scope of care provided by the affiliate, including but not limited to contraception, sexually transmitted diseases (including HIV) prevention and treatments, health promotion activities, pregnancy options, gender affirming hormone care, or abortion services. Providing appropriate and medically accurate patient education in support of all services offered by the affiliate.

Checking and responding to messages and communications via printer, fax, email, and EHR tasks. Triages patient calls appropriately based on affiliate protocols. Respond to patient calls by the end of the business day. Responding to MyChart messages within 2 business days. Expediting urgent/emergent calls as needed. Receive, manage, and direct calls in accordance with call management protocols, including but not limited to:
  • Routes calls to the appropriate department for continued assistance, as necessary.
  • Communicate requests and respond accurately using the appropriate procedures for contacting health centers and/or health center staff.
  • Maintain thorough knowledge and understanding of health operations to answer questions and direct calls correctly and appropriately.
Documenting interventions and education in the EHR of established patients using standing medical guidelines, abbreviations, and EHR documentation standards.

In conjunction with/at the request of the CFU Clinician Manager or clinician, assist the Education/Training Department in identifying clinical procedures and CFU processes in need of review for existing staff or new employees. Assists with training, precepting, and mentoring of new or existing staff. Acts as a medical resource for affiliate staff.

In addition to the responsibilities noted above, CFU APCs may be assigned to one or more Centralized Follow-Up APC and RN roles that entail tasks specific to that role, including:

High Level Follow-Up Clinician
  • Responsible for initiating and managing more complex follow-up (i.e., post-LEEP, post-vasectomy semen analysis, cancer diagnosis) and for tracking and managing urgent follow-up.
  • Interpreting and managing cervical dysplasia results (Pap smear, HPV testing, colposcopy, LEEP);
  • Initiating notification for more high-grade findings
  • Tasks to CFU RNs to initiate notification for low-grade findings
  • Managing cases that require urgent follow-up, including but not limited to ruling out ectopic pregnancy, emergency room referrals, acute procedure complications, and other conditions that require short term follow-up.
  • Managing bloodborne and non-bloodborne pathogen exposure incidents, completes documentation appropriately, and ensures outside records from occupational health evaluation are received and completed.
Centralized Follow-Up Clinician
  • Follows up with patients scheduled for abortion procedures who may have a medical condition that could affect their ability to complete the procedure at the affiliate (i.e., AB alerts);
  • Reviewing all abnormal lab results and/or imaging reports not related to urgent care or within the responsibility of the CFU RNs; initiates/manages follow-up and notifications to patients as appropriate.
  • Reviewing non-urgent incoming medical records, abstracts and summarizes them into the EHR, updates any relevant follow-up plans as needed, and coordinates care with the patient as needed.
  • Managing long term/non-urgent follow-up of abnormal breast findings, including imaging results.
  • Assisting the Prior Authorization Navigator with the Prior Authorization process as needed.
CFU RN
  • Performing reproductive health and STI services within the scope of her/his/their license. Services provided include notifying patients of and prescribing treatment for STIs and other minor conditions.
  • Completing mandatory reports for State Department of Health reportable conditions.
  • Assisting with completing Prescription Drug Monitoring Program queries, if requested by clinicians and/or physicians.
Triage RN
  • Performing reproductive health and STI services within the scope of her/his/their license. Services provided include contraceptive method counseling, refilling of medications per approved affiliate protocol.
  • In conjunction with the clinician, performance of triage according to affiliate protocol for new and established patients to ensure a timely appropriate response to patient concerns, medical needs, and access to care.
Minimum Qualifications:
Minimum two years’ experience as an ARNP required.
Current RN and ARNP licenses in Washington State.
Must reside in Washington State.
ARNP and specialty certification required.

Preferred Qualifications:
Women’s Health or Reproductive Health preferred.
Bilingual English/Spanish skills preferred.

Salary Range:
$48.51 - $90.10 hourly
