Position Salary Range $81,573.03 - $103,334.28 Per Year
Starting PayAlthough our full pay range is included above, the budgeted hiring range for this position is $81,573.03 - $92,453.65 Per Year.
Upon passing a Conversational Spanish Oral Exam, you may also earn an additional $1.25/hr.
This position is salaried, exempt.
Typically our work schedule is Monday - Friday 9am - 5pm
Remote Work: This position is 100% remote unless future legal requirements mandate on-site presence. We will actively monitor and stay compliant with any regulatory changes that may impact remote work eligibility. If laws change, providers may be required to transition to an in-person setting.
The Objective: The Integrated Behavioral Health Provider (BHP) is responsible for delivering behavioral health interventions to support PPRM patient health outcomes in a co-located behavioral health model. This involves collaborating closely with patient facing staff at PPRM and Clinical leadership. As a vital member of the interdisciplinary patient care team, the BHP provides guidance on administering and interpreting results of behavioral health screenings, conducting behavioral health assessments, supporting behavioral health crisis interventions, and coordinates behavioral health care, including providing up to six therapeutic sessions. This role is a remote position, with the opportunity to work virtually in a telehealth setting. Ultimately, the BHP will be the lead on determining the most suitable care pathway for PPRM patients seeking behavioral health support. Ensuring seamless continuity of care for PPRM patients through effective care coordination and linkage to extended behavioral health services is a primary focus of this position.

Qualifications
- You must have a master’s degree in social work, Counseling or clinical mental health counseling and have an active Colorado license in clinical social work (LCSW) with the ability to practice independently without clinical supervision.
- You must be willing to apply for and maintain licensure in New Mexico and Wyoming. Application will be facilitated by PPRM as needed.
- You have no restrictions on the ability to be credentialed with Medicaid or other payers
- You have no stipulations or complaints on clinical licensure
- You have at least two years’ experience in providing behavioral health interventions.
- You are experienced in working with diverse populations, including LQBTQ populations and supporting gender affirming care.
- You have strong interpersonal skills and the ability to collaborate effectively with diverse teams.
- You have excellent organizational and time management abilities.
- You are experienced in discussing sensitive information and education while reducing stigma required.

- Provide Outstanding Patient Care: At its core, your work is about providing care to patients who need you and doing so with the highest degree of competence and quality. You will triage behavioral health questions and concerns, in accordance with PPRM’s Medical Standards and Guidelines. You will provide crisis intervention and therapeutic intervention and will be a professional, nonjudgmental source of support to patients by offering information on reproductive, sexual health issues and gender affirming care both on site and/or by telemedicine. Additionally, you will collaborate with health center teams, including medical providers, to facilitate screening process and interpret results accordingly. Provide comprehensive behavioral health assessments and diagnosis where appropriate (in accordance with the DSM-V) using, provide effective treatment planning, therapeutic intervention, and assist clients in successfully achieving goals. As an experienced clinician with special populations, including those seeking gender affirming health treatment and family planning procedures, you will evaluate crisis situations and apply appropriate interventions. When indicated, you will make appropriate referrals to outside entities, including behavioral health specialists and others, to provide additional support, especially for the care of patients with complex needs like mental health, substance use, and other medical concerns. You will also develop and maintain professional relationships with patients to assist in the success of individual patient of treatment plans including providing appropriate follow-up care. You will provide excellent customer service to both internal and external customers by working towards a positive outcome with any problems encountered. You will perform mandatory reporting to Colorado, New Mexico and Wyoming State Departments of Health, when indicated.
- Support Capacity-Building Efforts Internally and Externally: You will be a vital part of the behavioral health team in assisting with the growth and development of PPRM’s Behavioral Health program. You may perform peer audits and provide feedback to others on your team. You will ensure compliance with regulatory standards and clinical best practices in behavioral health care delivery while cultivating a positive and inclusive culture. You will provide relevant education and collaborate with heatlh center teams and other community agencies in providing integrated behavioral health care. You will proactively and positively respond to change and others’ feedback. You will function with a low level of supervision.
- Communication: You will demonstrate and facilitate excellent written and verbal communication as well as organizational skills. You will have a knowledge of computer-based healthcare information systems and will utilize electronic health records and other systems to accurately document all interactions with clients, providers, and stakeholders in a timely manner. You will be able to manage technology independently.
