Job Description
As a Case Manager, you will oversee a designated group of patients and ensure they receive the appropriate level of care throughout their stay. This role involves collaboration with various healthcare professionals to coordinate treatment plans, facilitate communication among stakeholders, and promote positive patient outcomes.
Requirements
Responsibilities :
- Manage insurance authorizations, ensuring that necessary approvals are obtained for services rendered.
- Work closely with our Discharge Planner, ensuring patients are prepared for their transition to home or other care settings.
- Document patient progress and communicate updates to the healthcare team.
- Reviewing proposed treatments and procedures to ensure they are medically necessary and aligned with the managed care plan, including obtaining prior authorizations when required.
- Reviewing medical claims for accuracy and appropriateness, addressing any potential issues related to billing or coding.
- Conducting assessments of patients’ needs and developing individualized care plans.
- Coordinating services and resources required for patient care.
- Monitoring patient progress and modifying care plans as necessary.
- Facilitating communication among doctors, nurses, patients, and families.
- Ensuring compliance with healthcare regulations.
