Duties & Responsibilities:
- Resident Care & Assessment:
- Conduct thorough assessments of residents' health status, including physical, mental, and emotional well-being.
- Develop and implement individualized care plans for residents based on their health needs and goals.
- Monitor residents' health conditions, document changes, and communicate significant findings to the physician and other members of the care team.
- Administer prescribed medications and treatments, ensuring accuracy and compliance with medical orders.
- Provide wound care, IV therapy, and other advanced medical treatments as required.
- Perform routine medical procedures (such as taking vitals, administering injections, and dressing wounds) in accordance with care protocols.
- Supervisory & Leadership Responsibilities:
- Supervise and support the nursing staff, including CNAs and LPNs, ensuring that all care duties are carried out efficiently and effectively.
- Provide guidance and training for nursing staff to ensure compliance with facility protocols and high standards of care.
- Participate in staff meetings and collaborate with the leadership team to maintain optimal resident care.
- Communication & Collaboration:
- Effectively communicate with residents, families, and the healthcare team to ensure clear, accurate, and timely updates on patient conditions.
- Educate residents and their families on care plans, treatment options, and preventative health measures.
- Work closely with physicians, dietitians, social workers, and other healthcare professionals to coordinate care and enhance the resident experience.
- Documentation & Compliance:
- Maintain accurate, up-to-date medical records and documentation for each resident in compliance with regulatory standards.
- Ensure adherence to infection control protocols and all state and federal regulations regarding patient care and safety.
- Participate in regular care audits and review processes to ensure quality improvement.
- Emergency Response & Crisis Management:
- Respond to medical emergencies promptly and effectively, ensuring that residents receive the appropriate emergency care and intervention.
- Provide immediate interventions and stabilize residents in critical situations, following established protocols.
Education & Experience Requirements:
- Education:
- Associate’s degree in Nursing (ADN) required; Bachelor’s degree in Nursing (BSN) preferred.
- Graduation from an accredited Registered Nursing program.
- Licensure & Certifications:
- Active, unencumbered Registered Nurse (RN)/Licensed Practical Nurse (LPN) license in the state of Iowa.
- CPR and First Aid certification (or must be obtained upon hire).
- Additional certifications (e.g., wound care, IV therapy) may be required or preferred depending on specific duties.
- Experience:
- Previous experience in a nursing role, preferably in long-term care, skilled nursing, or geriatric care settings.
- Experience in supervisory roles and leadership is a plus.
Physical Requirements:
- Ability to stand, walk, and move for extended periods of time during shifts.
- Ability to lift and assist patients, up to 50 lbs or more, including moving and repositioning residents.
- Ability to bend, stoop, squat, and kneel to provide patient care, administer treatments, and assist with activities of daily living.
- Fine motor skills required for administering medications, performing clinical procedures, and handling medical equipment.
- Ability to communicate effectively with patients, families, and healthcare staff.
- Ability to react quickly in emergencies, providing immediate care and stabilizing residents.
- Good vision (with or without corrective lenses) for reading medical records, monitoring patients, and administering treatments.
- Emotional stamina to handle stressful situations, particularly in emergencies or with critically ill residents.
