SUMMARY Responsible for providing direct nursing care as ordered by physician, outlined in the plan of care, and in accordance with standards of nursing practices and regulations. Takes charge in leading and directing resident/patient care on assigned shift. Directly supervises Licensed Practical Nurses and Certified Nursing Assistants to ensure assignments and care are complete based on professional standards.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties as assigned. ? Personally participates in the assessment and delivery of care including medication administration, treatments, Adl care, etc. as ordered by physician and outlined in the plan of care. Performs all direct care outside of LPN scope of practice while remaining within RN scope of practice. ? Conduct regular rounds to monitor resident activity and ensure resident/ patients receive quality care based on professional standards and facility policies ? Assess residents physical and psychosocial status ? Communicate with residents, family members, physicians and supervisor regarding resident status. ? Ensure the medical records are maintained up to date and accurate during assigned shift ? Ensure all documentation assigned for clinical staff is complete prior to the end of the shift ? Coordinate and prepare for new admissions, transfers, discharge of residents ? Transcribe physician orders ? Assist in updating the plan of care that addresses the needs of the residents/ patients ? Complete incident/ accident reports ? Understands, complies with and promotes all rules regarding resident rights ? Maintain safe and clean work area ? Ensures proper storage of equipment and supplies ? Ensure Infection prevention and control practices are followed on assigned unit ? Report hazardous conditions, incidents and accidents, and allegations to Director of Nursing and/ or Administrator ? Ensure ordered diagnostic test are completed and followed up ? Complete scheduled assessments ? Delegate appropriate duties assignments to Charge nurses and Certified Nursing Assistants ? Assist in orientation and training ? Attend orientation and in-services ? Attend staff meetings ? Ensure adequate staffing during assigned shift. ? Call in off duty staff to replace any call ins for assigned shift and upcoming shifts ? Maintain positive working relationship and open communication with Director of Nursing ? May also be assigned title “Transitional Care Unit Director” (see Transitional Care Unit Director job description) ? Perform other duties as assigned or designated SUPERVISORY RESPONSIBILITIES Responsible for the supervision of the total nursing activities and care provided during his/ her tour of duty. QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE ? Associate Degree Nurse Diploma ? Long Term Care/ Skilled Nursing Care experience preferred CERTIFICATIONS, LICENSES, REGISTRATIONS ? Must be a Registered Nurse in good standing and currently licensed by the state ? CPR-BLS ? Valid State issued Identification KNOWLEDGE, SKILLS AND ABILITIES ? Excellent written and verbal communication skills ? Ability to multi-task while being detail oriented ? Intermediate computer skills
