GENERAL FUNCTIONS:
Facilitates the daily operation of the Admissions Office in accordance with current Pleasant Ridge Manor (PRM) policies and procedures. Serves as the PRM liaison to hospitals and other community health care facilities and agencies. Promotes PRM to the community as a resource for skilled nursing care. DUTIES AND RESPONSIBILITIES:
- Plans, develops, organizes, implements the activities of the Admissions Office.
- Coordinates Admissions Office activities with other related departments.
- In co-ordination with the Admissions Committee, maintains and updates Admissions-related policies and procedures.
- Performs administrative duties necessary to facilitate effective functioning of the Admissions Office.
- Participates ongoing in the Quality Assurance program for the Admissions office/Social Services Department.
- Participates in surveys (inspections) made by authorized governmental agencies.
- Meets with Administration, Medical and Nursing staff, as well as other related departments in planning and facilitating Admissions.
- Serves on, participates in, and attends various committees of the facility as required and/or directed.
- Participates in meetings as directed.
- Responds to all inquiries and referrals for admission, via in-person appointments, phone contacts, and electronically.
- Gathers information needed to facilitate admissions, including but not limited to demographic, medical, psychosocial, and financial.
- Coordinates communication with other departments as needed to facilitate admissions, including but not limited to assuring a timely review of pre-admission records by appropriate departments.
- Arranges for the admission of new residents, including but not limited to securing administrative approval, entering appropriate information to begin a Medical Record, providing information to the Medical records Department for processing, notifying all appropriate departments of planned admission, assuring that any insurance authorization or other needed authorizations are secured prior to admission, completing required state forms such as the PASSR-ID.
- Meets with the new resident and/or responsible party to explain the Basic Service Agreement and the other forms which are included in the Admission Packet. Secures appropriate signatures and processes the Admissions Packet as per current policies and procedures.
- Obtains updated demographic, social history, discharge planning and other information of new residents, and enters this into the Medical Record as per current policies and procedures.
- Proactively participates in growing relationships with area hospital discharge planners and other community resources, and communicates the services and benefits offered by Pleasant Ridge Manor to potential residents and families.
- Maintains and updates logs for admissions and referrals, as per current policies and procedures.
- Completes monthly and yearly Admissions reports as directed.
- Provides point of contact to local hospitals to coordinate information and needs of hospitalized residents, including but not limited to coordinating/sharing updated medical information, and coordinating residents returning from the hospital.
- Keeps up to-date on professional information that is necessary to be helpful to potential residents, families, and community health care professionals.
- Ensures that all needed supplies for the Admissions Office are maintained.
- Performs other related duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES:
Authority to Exercise Decision Making:
Exercises high level of decision-making responsibility and authority with regards to Coordinating referrals for admissions, and sharing information. Consults with other Department Heads and/or Administration on decisions affecting other areas and/or of a specialized nature.
EDUCATION AND EXPERIENCE REQUIRED (REQUISITES):
Education:
· Bachelor’s Degree in Social Services, Health-related field or Marketing preferred
Experience:
- Two (2) years’ experience in a health care facility, preferably in a skilled nursing environment.
Job Type: Full-time
Pay: $20.00 - $22.00 per hour
Expected hours: 37.5 per week
