Job Description

Job Title : Licensed Practical Nurse (LPN)

Job Specialty : Long-Term Care / Skilled Nursing Facility

Job Duration : 15 weeks

Shift : 8-hour evenings, 2 : 45 PM - 11 : 00 PM, every other weekend

Guaranteed Hours : 38 hours per week

Experience : Minimum 1 year in Long-Term Care

License : LPN

Certifications : Basic Life Support (BLS)

Must-Have :

Prior experience with PCC (PointClickCare)
Experience measuring wounds

Job Description :

Provide care and administer medications and treatments for residents 18 years or older in a long-term care, skilled nursing, rehab, or dementia setting.

Perform blood glucose monitoring, insulin administration, IV medications (primarily antibiotics, Lasix, and Bumex), and manage TPN and tube feedings.

Execute wound care, including measurement, conduct lab draws and specimen collection, and provide foley and catheter care.

Conduct tracheostomy care, assistance with activities of daily living (ADLs), assessment charting, and incident reporting.

Monitor vital signs and manage oxygen administration and equipment, including CPAP and BiPap.

Work under the supervision of an RN supervisor and with a multidisciplinary team including PT, OT, SLP, case managers, social workers, and dieticians.

Attend care plan meetings occasionally, particularly for short-term stay residents.

Use PCC for electronic medical records and contribute to a collaborative team environment for patient care.

The position requires a willingness to float between units as needed, adherence to a professional dress code, and compliance with scheduling and holiday requirements.

Participate in a 3-day unit / department-specific orientation.
