What are the responsibilities and job description for the Day Program Coordinator position at PLEASANT VIEW INC?
Major Job-Related Duties and Functions:
Responsibilities include, but are not limited to, planning and scheduling activities, managing individuals' funds, providing medication management, and providing direct care. The PC oversees DSPs, assigns program tasks, addresses employee concerns, and ensures daily documentation is completed accurately. The PC is responsible for writing Individual Service Plans and quarterly reports. Additionally, The PC also participates in the administrative hospital on-call rotation.
Minimum Qualifications: (knowledge, skills, education, and abilities)
- Excellent verbal and written communication.
- Possess a current, valid driver’s license for three (3) years with an acceptable driving record.
- Ability to present information well to individuals and groups.
- Must be organized and detail-oriented.
- Must be able to maintain confidential information.
- Ability to work in a fast-paced environment, multitask, work in a team setting as well as independently.
- Excellent computer skills. Proficient in Office (Word, Excel, PowerPoint) and excellent data entry skills.
- Ability to access all facilities and areas of all the programs.
- Ability to push, pull, bend, reach, walk, stand for extended periods, climb stairs, lift and stoop, and to assist an individual to move as needed.
- Must be able to attend and pass all required training.
- Distribute medications to individuals following approved guidelines.
- Ability to work varied hours to cover program needs.
- Must be able to perform the essential functions for all direct reports and pass and be competent in DSP supervisory competencies.
- Ability to serve on the administrative emergency on-call rotation
- Ability to adhere to all safety rules, policies, and guidelines.
- Ability to become familiar with, uphold, and promote the mission, vision, values, and policies of Pleasant View, Inc., including Home and Community Based Services (HCBS) regulations
