Job Title : Charge Nurse

Department : Nursing

Supervisor : Director of Nursing

Summary :

The primary purpose of your position is to assist in the planning, developing, organizing, implementing, evaluating, and directing the nursing services for our unit, while delivering on the core values of wellness, compassion, customer experience and company results.

Essential Duties and Responsibilities include the following :

Adherence to Policies :

  • Must show courtesy and respect to other employees, residents, family members, visitors, government agencies, the general public, etc.
  • Abide by Eduro HealthCare’s drug and alcohol policy. Must not be a habitual user of drugs (either prescription or non prescription) or alcohol.
  • Maintain good personal hygiene and dress and groom appropriately.
  • Report on time, prepared to work as scheduled and follow all company policies and procedures.
  • Attend staff meetings and in-service sessions.
  • Become thoroughly familiar with emergency procedures.
  • Must be able to perform duties in a timely fashion, and within the prescribed sequences and schedules.
  • Must be able to perform assigned tasks with a minimum of supervision and develop awareness for and a willingness to perform other tasks, as needed, without supervision.
  • Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.
  • Follow all established safety precautions when operating equipment, and report hazardous and defective equipment or conditions to your supervisor.
  • Must be cooperative with other departments and be courteous and respectful in dealing with them at all times.
  • Participate in and respond professionally to surveys (inspections) conducted by government agencies.

Patient Care :

  • Understand and implement the plan of care and assess resident needs. Periodically review the care plan for appropriate resident goals, problems, approaches, and revisions based on nursing needs.
  • Plan for the care of each resident upon discharge by working with the physician, family, social services department, and other health-related agencies as appropriate.
  • Respect all resident rights, including the confidentiality of resident care information.
  • Must report immediately to the Administrator and to the proper legal authorities if you have reason to believe a resident has been physically, emotionally, or sexually abused, or been a victim of theft of their personal property.
  • Communicate with residents and families to gather information relevant to the resident care plan.
  • Create and maintain an atmosphere of warmth, cheerfulness, enthusiasm, and love, giving the resident the quality of service you would want to receive personally. Reflect the four core values of Eduro Healthcare.
  • Other Duties :

  • Assume the authority, responsibility and accountability necessary to carry out the duties of this job.
  • Receive report at the beginning of each shift. Assess the physical and emotional status of each resident and evaluate their need for continued or immediate care.
  • Delegate responsibility for the direct care of each resident to appropriate nursing personnel according to needs, physical layout of care areas and staff ability.
  • Be responsible for all phases of direct resident care, within the guidelines of the Nurse Practice Act.
  • Ensure that nursing staff perform all restorative measures recommended in the plan of care and required by each resident’s condition. Report all incidents of change in resident’s condition to the Director of Nursing.
  • Assist physicians on rounds and keep them informed of the status of each resident, changes in condition and medical emergencies.
  • Plan working schedule to allow for education, supervision, and evaluations of performance for all nursing personnel during the shift.
  • Maintain completeness of resident charts and individual care plans, making sure entries are timely, informative and descriptive of resident’s condition.
  • Write daily nursing notes on all skilled residents, and monthly summary notes on each resident according to posted schedule.
  • Be responsible for timely execution of all physician orders, including laboratory testing, specialized therapy orders, and preparation and administration of all medications as indicated.
  • Ensure that Care Center policies and procedures are applied and respected by all employees during their shift.
  • Collect specimens for testing, according to physician orders, and fill out necessary laboratory request slips as well as secure laboratory reports in a timely fashion.
  • Be responsible for the maintenance and upgrading of personal nursing skills through regular attendance at in service sessions, community classes and workshops.
  • Count narcotics with the oncoming nurse, as needed, to ensure accurate report.
  • Give report to oncoming shift nurse.
  • Document weekly on each D.Q. / skin condition sheet and summary.
  • Check all charts of residents admitted within the last week for completeness and finish documentation as needed, including, the admission history and assessment, MDs, care plan, AIMS assessment, and fall assessment.
  • Chart the intake and output summary for each resident on the appropriate monitoring form.
  • Qualifications :

    To perform this job successfully, an individual must be able to perform each essential function satisfactorily.     Education and Experience : Must have graduated from an accredited school of nursing and have a current license to practice as a Registered Nurse in the State of Utah.

    Language Skills :

  • Must possess the ability to work well and communicate effectively with other employees, residents, family members, visitors, government agencies, the general public, etc.
  • Must be able to read, write, speak, and understand English sufficiently to perform required duties. Bi-lingual language skills helpful.
  • Must be able to follow verbal and written instructions in English.
  • Reasoning Ability : Must be perceptive, with good judgment and decision-making / problem-solving skills.

    Certificates, Licenses, Registrations :

  • Completion of orientation required by company policy.
  • Mandatory drug test within 90 days of hire.
  • Must be free of criminal activity proven by a criminal background check.
  • Become familiar with and comply to all local, state, and federal regulations relating to the job.
  • Show within three (3) days of hire satisfactory evidence of identity and eligibility for employment.
  • Previous experience as a Medication Nurse and experience in working with geriatric residents is preferred and may be required.
  • Physical Demands :

    The physical demands described here are representative of those that must be met by an employee to    successfully perform the essential functions of the job. The employee must be able to perform the essential job functions without posing a direct threat to residents, self or others.

  • Must be able to sit, walk, lift and carry in excess of 75 pounds, bend, climb, kneel, squat, stoop, push, pull, grasp, reach arms above head, and have hand and finger dexterity.
  • Must be able to rapidly assist in the evacuation of all residents from the building in case of emergency.
  • Must be able to cope with the mental and emotional stress of working daily with geriatric residents and dealing with emergency situations.
  • Must be able to see, hear, and smell sufficiently to assure the requirements of this position can be fully met.
  • Must be in good general health and be emotionally stable
  • Must be able to perform the essential job functions without posing a direct threat to residents, self or others.
  • Equipment :

    Must be able to operate the following equipment :   fire extinguisher, blood pressure cuff, stethoscope, weight chair, Hoyer lift, thermometer, scissors, restraints, whirlpool bath, gait belt, hair dryer, curling iron, needles, IV equipment, medicine cart, supplies cart, nurse chart tray, electric beds, wheel chairs, traction equipment, oxygen equipment, intubation equipment, computer, telephone, etc.

    Work Environment :

    Must possess the ability to work in a wet or humid environment. Employee should be willing to work with body fluids, excretions, extreme odors, etc., while using universal precautions. Employee understand they may be subject to the exposure of infectious waste, diseases, conditions, etc., including the AIDS and Hepatitis B viruses. Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
