PMHCC - CTT (CTT) is mandated to provide service to those individuals within the mental health system who present the greatest degree of severity of symptoms as evidenced by their multiple physical, psychiatric and legal conditions; extensive use of services and lack of follow through with treatment. These individuals measure at the highest level of medical necessity as defined by the state Adult Environmental Matrix.
The Acute Care ACT Team is a specialized team that provides multidisciplinary clinical review and assessment of individuals actively involved in the criminal justice system but may also be heavy users of services, young adults or may also have co-occurring behavioral health diagnoses. The ACT Team Leader collaborates closely with DBH, CBH, TCM, judicial and other systems around comprehensive needs assessments of these individuals, makes recommendations and provides supervision and direction to staff. The ACT Team Leader is responsible for ensuring that the mission, goals and philosophy of the CU are operationalized within the team. The incumbent encourages a strengths-oriented treatment/service planning that addresses the individual’s principal needs, the incumbent works with staff to ensure continuity of care and enhancement of individual’s understanding, acquisition and maintenance of independent living skills in the areas of: (a) their daily living situation; (b) interpersonal skills and social support/network building; (c) leisure and recreational support/skill development; (d) maintenance and enhancement of physical and mental needs; (e) obtainment of benefit entitlements and the skills to manage same; (f) vocational and educational needs; (g) accountability in fulfilling any legal obligations in order to remain in the community.
Duties and Responsibilities
- Manages team, coordinates and directs activities of all team staff in order to ensure necessary services are delivered to team’s caseload; provides group supervision to team weekly and direct supervision to clinical, CM, Program Assistant staff members every two weeks; also supervises CPS staff every two weeks or in conjunction with CPS Coordinator when necessary; provides at least 20% direct clinical service as required by ACT standards.
- Provides daily oversight/coordination to multiple interdisciplinary team internally (including psychiatrists, psychologist and nurses) to ensure a comprehensive approach to treatment; collaborates with external teams to ensure coordination between systems.
- Provides assistance in viewing individuals from a strengths approach in preparation of Comprehensive Service Plans and Participant Schedules in conjunction with Individual Treatment Team process, including training CM staff on the development of both.
- Monitors service delivery to individuals to ensure appropriate level of service given, as defined by Environmental Matrix score, and in accordance with fidelity to ACT Model.
- Establishes networks and liaisons with community service providers to resolve problems and overcome barriers to service, including attending linkage and treatment team meetings as needed; provides advocacy mechanism to ensure that individual needs are met.
- Provides in field Side by Side Supervision bi-annually for all non-administrative staff persons.
- Manages fiscal areas including: a) authorization of expenditures for individuals with CU representative payee status; b) approval of monthly staff travel expenses and usage of public transit tokens.
- Maintains and updates community resource file, particularly around young adult populations or heavy users of services and provides information and access to staff and individuals assigned to team.
- Conducts daily team meetings to provide group supervision; monitors staff daily schedules to ensure that tasks are evenly distributed and properly followed through; establishes a system of communication to ensure information flow and safety of staff and effective response when needed; maintains an information log of individuals discussed at each meeting.
- Assigns caseload and team tasks to team members; provides conflict resolution and tactical suggestions in regard to disagreements to treatment approaches that arise within the team and other agencies.
- Monitors the completion of and sign-off on all required documentation; trains and supervises staff in the completion of all required documentation; provides record of team productivity and outcome data as required; completes required monthly reports and other documentation in timely manner.
- Maintains compliance with all applicable behavioral health treatment standards as set by state and county funding authorities, and ensures that agency standards, policy and procedures are adhered to by team members.
- Participates in Management, Clinical Care, Team Leader, Open Forum and other required agency and external meetings; attends in-service and other training in order to meet mandated training hours.
- Plans and assists individuals to increase community tenure, enhance quality of life and attain highest level of independent functioning.
- Provides 24 hour/7 day a week access to service through participation in weekly on call rotation schedule, allowing for evening/weekend hours as required to respond to consumer needs.
Skills Required:
Must have excellent verbal and written communication skills, ability to work well with people, have good, creative problem solving skills; excellent clinical skills including assessment, knowledge of behavioral health and criminal justice systems, community resources, networking and supervisory skills; computers skills; be able to work independently. flexible and adaptive in handling changing priorities in a fast paced work environment, able to work extended hours as may be required to complete projects.
Education and Experience:
Master’s degree in (including but not limited to) social work, psychology or counseling with at least five (5) years of experience in clinical assessment and therapeutic work, two (2) of which includes supervisory experience. Must have received Certification as a CPS Supervisor, or receive within 6 months of hire. Expertise preferred in behavioral health and one (1) or more of the following: transitioning youths, criminal justice and substance abuse, including training in PCPC, HIV/AIDS, etc.
Must have a valid driver’s license, auto insurance and vehicle; Must have ability to work evenings and weekends in meeting on call responsibilities.
