Job Description - Peer Navigator - POCAAN
HIV POCAAN seeks a qualified individual to fill our Peer Navigator position.Weare seeking someone living withHIVand who understands how to navigate the care systems and services for people living with HIV. Interestedindividualsshouldbe willing toshare some oftheir ownstorieswith clients tohelp build rapport.

POSITION SUMMARY
ThePeerNavigator,underthegeneraldirectionofthePeer Supervisor,willprovidesupport &carecoordination services, education, and advocacy for individuals living with HIVwho arebeing releasedfrom jailsto the community. ThePeer Navigatoris a person living with or affected by HIV and serves as a role model and a guide for other people living with HIV. They help provideaccuratehealth information, navigate care systems, help clients understand HIV care best practices, and serve as a personal resource to newly affected HIV clients.Thiswill bea full-time position with flexibleworkinghours.
As apeer navigator, theperson in this positionwill help form peer alliances withclients.
RESPONSIBILITIESANDDUTIES :
- DemonstratesanunderstandingofissuesrelatedtohealthcareandHIV.Persons living withHIVwho have had experience with theincarceration systemisstronglydesired.
- Identifyresources and support services for clients & facilitatereferrals asneeded.
- Work with the client to carry out the care plans developed between the clients & careteam
- Meet with clients shortly after release fromjail
- Act as a buddy / navigator forclients
- Collaborate with clients toestablishor re-establish a medical home by making medicalappointments.
- This job will require frequent travel within King County
