Location: Portland, OR 97216
Hours: Monday - Friday, 8:00am - 4:30pm
Reports to: Mental Health Director
Salary: $20-$26/hour

Job Summary
This position is responsible for working closely with POIC Mental Health Professionals and other staff to identify students, participants, and families who would most benefit from health navigation support and case management to make the best health care decisions based on their individualized needs. This could include helping them schedule appointments with internal or external health care providers, connecting them to nutrition resources, and supporting them in overcoming barriers to quality care such as a lack of insurance, transportation, or childcare. The PHN will collaborate with healthcare teams to improve participant care strategies and support participants to receive timely, effective, and coordinated care by performing the following duties.
Essential Duties
Provides person and community-centered care to bridge communities and the community support systems that serve them.
Enhances community health and broadly promotes health and wellness by conducting ongoing needs assessments, building relationships, and systematically seeking to reduce stigma.
Provides a wide range of support services for students and families.
Utilizes a culturally responsive trauma-informed and healing centered approach in supporting students and families.
Supports students, families, and POIC participants in navigating safety net and health care systems to access needed services. Works with students, families, and POIC participants to reduce barriers to care and advocate for their needs.
Provides advocacy, outreach, case management, coordination of care, and support participants to understand their healthcare needs.
Coordinates care and appointments between different healthcare providers.
Works closely with students, families, and POIC participants to promote health education, prevention, and wellness, while addressing social determinants of health and fostering trust within the community.
Provides culturally sensitive support and education to students, families, and POIC participants related to health, wellness, climate resiliency, and community resources.
Collaborates/coordinates with other POIC staff, caregivers, and other involved providers.
Establishes and maintains effective working relationships with youth and families of color.
Keeps accurate, confidential record of case notes documenting sessions and adheres to high professional and ethical standards.
Seeks out opportunities for further training in case management, traditional health work, health navigation, climate resiliency or other related topics.
Meets periodically with staff to obtain feedback for case management, while maintaining confidentiality.
Performs other duties as assigned.
Experience/Education
Formal education is not a requirement of this role. This position requires three years related experience. An equivalent combination of education, training, and experience may be used to satisfy these requirements.
