This position is responsible for providing ongoing case management to high-need, high-risk members diagnosed with SUD, including those with medical comorbidities and social determinants of health that make acceptance into care challenging.
About the Role
- Provide targeted health education and proactive strategies for condition management.
- Facilitate program enrollment utilizing key motivational interviewing skills.
- Perform telephonic and face-to-face outreach to assess barriers to wellness.
- Serve as an internal subject matter expert on SUD treatment, resources, regulations, and outside agencies.
Key Responsibilities
- Administer assessments and surveys to develop a plan of care.
- Collaborate with members/caregivers and providers to implement member-specific care manager interventions.
- Evaluate plan of care and revise as needed.
Requirements
- Licensed Drug and Alcohol Counselor (LDAC) required.
- Associate's degree or above required.
- 3 years of experience in addictions or behavioral health setting working with individuals in recovery.
