Main Responsibilities:
- Serve as a Liaison: Connect members with their health plan nurse and facilitate smooth onboarding and ongoing support.
- Conduct Telephonic Outreach: Follow up with prospects who expressed interest and review eligibility for MassHealth and the THP SCO program.
- Partner with Community Resources: Collaborate with community partners, such as Community Health Centers, Providers, and Social Service organizations, to facilitate prospect and member engagement.
Key Skills and Qualifications:
- Strong Communication: Possess excellent written, oral, and interpersonal communication skills to convey complex information effectively.
- Cultural Competence: Work compassionately and efficiently with diverse populations, promoting empathy and understanding.
- Experience with CRM Systems: Proficient in using Customer Relationship Management (CRM) systems to manage interactions and track progress.
