What are the responsibilities and job description for the Intake Specialist - Behavioral Health position at Pokagon Band of Potawatomi?
Pokagon Health Services has a part-time opening in our Behavioral Health department for an experienced Intake Specialist. This is a part-time position with a hybrid work schedule. This position is subject to pre-hire and random drug and alcohol screening including marijuana.
Position Summary: The Integrative Care Specialist is responsible for the transition and referral coordination between primary care and behavioral health services.
Essential Functions:
- Establish rapport quickly and to communicate effectively with patients of healthcare, their family members and providers.
- Function effectively as a member of an inter-professional team that includes behavioral health, primary care, pharmacy, outreach, and social work providers.
- Conduct brief, evidence-based and developmentally appropriate screening, assessments, and questionnaires. Examples include screening for risky, harmful or dependent use of substances, cognitive impairment, mental health problems, behaviors that compromise health, harm to self or others, abuse, neglect, and domestic violence.
- Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety. This tracking will use an IT database.
- Assemble information concerning patient's medical history, mental health history, and referral needs.
- Review details and expectations about the referral with patients.
- Assist patients in problem solving potential issues related to the health care system
- Act as the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns. May assume advocate role on the patient's behalf to access additional services.
- Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providers.
- Ensure that referrals are addressed in a timely manner.

Position Requirements:
- Bachelor’s degree in health care related field required. Degree must be from an accredited institution whose program of study has been recognized by the Council for Higher Education Accreditation or the United States Department of Education.
- Previous experience in behavioral health preferred.
- Ability to alleviate stress and anxiety and be supportive of persons and work tactfully, collaboratively, diplomatically and with highest level of confidentiality required.
- Excellent communication and interpersonal skills required.
- Proficiency with MS Office Applications preferred.
- Experience working with Native American communities; knowledge of Native American traditional practices and values and Native American issues preferred.

Physical Requirements:
- Work is generally sedentary in nature and will frequently require sitting, however standing and walking will occasionally be necessary.
- Frequently required to talk and hear.
Work environment:
- General office environment. Lighting and temperature are adequate, and there are no hazardous or unpleasant conditions caused by noise, dust, etc. Occasionally work environment will be in other settings, including hospitals, homes, health clinics, schools, jails, and other public facilities. Local travel is necessary, so the ability to travel as needed is required.
