Responsibilities

- Develop, implement, and evaluate individualized care plans based on patient needs.
- Monitor patients' vital signs and report any changes in condition to the healthcare team.
- Administer medications and treatments as prescribed by physicians.
- Collaborate with interdisciplinary teams to coordinate patient care and discharge planning.
- Maintain accurate medical records using electronic health record systems such as Cerner.
- Provide education and support to patients and their families regarding health management.
- Participate in case management activities to ensure continuity of care.
- Uphold standards of managed care practices within the facility.
Skills
- Proficient in medical terminology and documentation practices.
- Experience with discharge planning processes and acute care settings.
- Familiarity with case management principles and managed care systems.
- Knowledge of medical coding practices for accurate billing and recordkeeping.
- Ability to work effectively in high-pressure environments such as ICU or emergency settings.
- Understanding of Minimum Data Set (MDS) assessments for skilled nursing facilities.
- Strong communication skills for effective collaboration with healthcare professionals and patients.
