Responsibilities

- Collaborate with interdisciplinary teams to coordinate patient care and discharge planning.
- Maintain accurate medical records using electronic health record systems such as Cerner.
- Provide education and support to patients and their families regarding health management.

- Uphold standards of managed care practices within the facility.
Skills
