Polaris Healthcare and Rehabilitation Center is a beautiful 100 bed skilled nursing facility located at Milford Wellness Village. We are currently seeking an experienced SOCIAL SERVICES to join our team!
Position Responsibilities for the SOCIAL SERVICES
- Supports the Social Services Director with department planning and organization necessary to ensure patient / resident medically related, emotional and social needs are maintained on an individual basis
- Acts as a liaison and representative of the residents’ social interests.
- Develops social assessment and care plan, which identifies medically related social and emotional problems and needs with realistic goals and specifications to be taken
- Will be actively involved in pre-admission and admissions processes
- Must have the ability to listen and act compassionately with residents / family members; must be able to work on own and maintain job related jobs
- Documents residents’ behavioral and psycho-social status
- Provides individual and group help for family and residents at all times of adjustment, crisis, or particular need
- Orients resident and family to the facility
- Develops and maintains good working rapport with patients / residents, family members, interdisciplinary team members, service providers and facility staff to ensure that social services programs can be properly maintained to meet the needs of patients / residents
- Participates in the admission and discharge planning processes, conducts patient / resident assessments, and care planning process
- Assist in maintaining department reports and assure records are survey ready
- Other duties as assigned
