What are the responsibilities and job description for the PER DIEM RN position at Polaris Healthcare?
Rate: $50-$150; Assignment based
RN NEEDED FOR PER DIEM;
- NYS Registered Nurse
- This is a position that would go between our facilities in the region
- Must have recent experience performing and entering UAS assessments for adult patients (preferably in an assisted living setting)
o Assessments must be done in person during hours that are acceptable to the home/residents
o Paperwork can be potentially done remotely
- Prefer to have experience doing 4449c, MD order and interim order paperwork
- Experience in the Assisted living setting strongly preferred
- Experience in doing UAS assessment to achieve RUG scores as it relates to physical/occupational/ speech therapy STRONGLY preferred
- Experience with OASIS assessments preferred but not required
Salary : $50 - $150
