MAJOR FUNCTIONS
The Community Outreach Specialist will plan, develop, and implement outreach marketing, education, enrollment, and promotional activities for the Community Health Care Program (CHC) across multiple markets, intersecting with the low income and indigent populations of Polk County for service connectivity. Assists with the marketing and promotion of Health & Human Service (HHS) programs and special events that promote the health and well-being for Polk County residents. Assists in coordinating outreach participation with internal team members.
Interacts and functions throughout the community using discretion and good judgement. Develop reporting criteria to measure effectiveness.
ILLUSTRATIVE DUTIES
Promotes CHC as well as other HHS programs through participation in health fairs, conferences, and networking with local health and social service community partner agencies. Nurtures ongoing relationships with collaborative partners strengthening the framework, reach and impact of knowledge, awareness, and the ability to navigate health and social services in Polk County. Represents division in multiple settings. Makes internal and external presentations related to health and social service programming. Identifies significant gaps in health and human service availability for citizens who are unable to secure affordable medical care, housing, transportation, and other social service needs, including but not limited to, the elderly, disabled, chronically ill, and disadvantaged families.
Participates in collaborative planning and grant activities with representatives of county divisions, other local agencies, and organizations as appropriate, and follows through to ensure that all activities are carried out through completion. Prepares, develops, (re)assesses, and carries out community outreach program activities throughout Polk County and reports on activity, engagement, and the ability to assist in connecting residents to services. Works with division managers, team leaders, and administration to ensure outreach aligns with division goals. Explores opportunities to promote programs in additional settings.
Assists with development of promotional materials including the use of Polk County Government Television, brochures, flyers, print materials, radio, and commercial advertisements. Responsible for the inventory and distribution of printed materials. Performs market research and data analysis to identify populations and areas for targeted promotions. Tracks and analyzes results from promotional activities. Assists clients with enrollment in Polk Health Care Plan and coordinates referrals to programs as needed.

SPECIAL REQUIREMENT
During times of impending or declaration of emergency (i.e. hurricanes, tornados, flooding), the person filling this position shall be required to report for work as directed and must be available to participate in the Polk County Emergency Management Plan. This may require the employee to be away from their home and family for several consecutive days.
