Shift : Monday - Friday, 8 : 00 AM - 5 : 00 PM (40 Hours / Week)

MAJOR FUNCTION

Responsible for team management of the Healthy Families Polk Project. The position supervises Family Support Workers and Family Assessment Workers. Responsible for monitoring and conducting quality assurance activities.

ILLUSTRATIVE DUTIES

Assists in interviewing, hiring, firing, and evaluating Family Support Workers and Family Assessment Workers. Recommends other personnel actions such as promotion, staffing, and discipline in accordance with County Policy. Coordinates, assists, and provides training of Family Support Workers regarding dynamics of child abuse and neglect, parent child interaction techniques, various curricula, and other required topics. Provides supervision, case management and monitoring of performance for the Family Support Workers and Family Assessment Workers. Responsible for in-service training for Family Support Workers on an on-going basis and provides in-service training as needed for the Family Assessment Workers. Maintains a work environment that provides maximum safety and health and encourages safety awareness among employees with an on-going unit safety program. Reports unsafe conditions and injuries / illnesses and incidents in a timely manner and ensures that employees perform duties using safe work techniques and in accordance with OSHA and County safety and health rules and sanitation standards. Familiarity with the County's policies, Healthy Families Polk policies, and Employee Handbook. Responsible and accountable for ensuring compliance and assists in meeting the Plans' goals and objectives. Responsible for ensuring that staff has materials and training needed to perform all aspects of their assigned duties particularly parent-child interaction facilitation and coordination with physician services for the project.

Directs Family Support Workers in arranging for assessments to be done with clients including the Individual Family Support Plan (IFSP), Ages & Stages Questionnaire (ASQ), Parent Stress Index (PSI), Home Safety Test and the Home Observation Measurement Environment / Infant-Toddler Home Inventory (HOME). Trains Family Support Workers on the techniques used by parents to improve deficit areas in parent-child interactions. Conducts weekly client record reviews and maintains required data collection systems for projects, including information for monitoring reports and project evaluations.

Implements quality assurance plan for Healthy Families Polk. Coordinates and ensures that Family Support Workers perform monitoring and linkage activities for all project families on an on-going basis.

Reviews cases on a regular basis and ensures that any case which may involve "imminent harm" or "threatened harm" is referred to CPS.

Works in conjunction with physicians, coordinates Public Health Nursing, hospital pediatrics and perinatal staff, and other providers of maternal-child healthcare and children's services. Ensures that all contract requirements are met, including defined service activities, target group numbers and designated client testing. Provides documentation on services needed by each project family and availability of needed services, as part of project assessment. Responsible for following proper universal precautions and infection control techniques per policy.

Maintains and demonstrates current knowledge and operation techniques on all pieces of equipment in the Unit utilized by the position. Assists with the Unit's Performance Improvement Program. Maintains and demonstrates competency in direct patient care of newborns, infants, children and adults. Attends required meetings, required in-services, and continuing education / seminar programs as requested to promote self growth and to stay current on issues involving Healthy Families Polk.

Performs other duties as required.

KNOWLEDGE, ABILITIES AND SKILLS

Knowledge of established County policies and procedures. Knowledge of the types of facilities and equipment utilized by the work unit. Ability to plan, assign, schedule, review and supervise the work of others. Ability to use good judgment in administrative and personnel situations. Ability to establish and maintain an effective working relationship with other employees, officials, and the general public. Ability to maintain a professional and safe unit and work area in order to adhere to established safety policies and the image desired for the organization. Ability to review and maintain data for projects, contracts, reports, and evaluations. Ability to communicate effectively both orally and in writing. Ability to provide clinical input on all clients and monitor the caseload of each Family Support Worker. Ability to plan, organize, and implement appropriate training and team building activities for the Healthy Families Polk Staff. Must be able to read, write and speak the English Language in an understandable manner. Must possess the ability to deal tactfully with employees, visitors, social service agencies, and the general public. Must possess sight / hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be met. Must be willing to work beyond normal hours and on weekends as necessary. Ability to lift, move, and maneuver equipment and supplies. May be exposed to safety hazards, infectious diseases including HIV and Hepatitis B viruses.

MINIMUM QUALIFICATIONS

Graduate of a four (4) year college or university with a degree in Human Services or a related field and have a minimum of three (3) years client service experience in a Human Services Program which included one (1) year supervisory experience in a Human Services Program.

Must possess a valid driver's license and be able to secure a valid Florida driver's license at the time of application.

SPECIAL PREFERENCES

Prefer a Masters degree in Public Health or a related Health / Human Services field. Experience working in maternal / infant health and child abuse and neglect preferred. Bilingual abilities a plus.

SPECIAL REQUIREMENTS

Applicant is required to complete a Level 2 background screening (FBI check) prior to employment. Applicant must also sign the Healthy Families Florida Standards of Confidentiality and Information Sharing Form, the Affidavit of Understanding form, The Department of Children and Families (DCF) Security Agreement form and the Affidavit of Good Moral Character form.

The Healthy Families Division is responsible for assisting in the Emergency Management Operations for Polk County in the event of an emergency such as a hurricane. The Healthy Families Division staff may be called upon to staff special needs shelters or assist with arranging transportation.

This position shall be required to report for work as directed when a declaration of emergency has been declared in Polk County.
