Crisis Stabilization Services Coordination

In this role, you are a member of our Transition and Coordination team (TRACS). The TRACS team is a specialized clinical service-focused group of Qualified Mental Health Professional (QMHP) level clinicians that work collaboratively to support individuals in stabilization and transition to longer term services.  

This position takes point on supporting adults in stabilization following a crisis intervention.   You will provide interim support to individuals referred by our crisis team by completing initial assessments, treatment plans and referrals. As a collaborative team member this position also provides back up support to the Civil Commitment Investigator and participates in case transfers and coverage within the team, providing flexibility to all team members and the ability to practice self-care, professional autonomy and have variety in your clinical practice role. 

WORK LOCATION

This position may be eligible for hybrid in-office/remote work as allowed by policy and approved by the supervisor upon completion of the employee’s trial service period.

BONUS ELIGIBLE

*Licensed candidates may be eligible for an additional 6% pay differential.

*Licensed MH providers who are also credentialed as a CADC may be eligible for up to an 8% pay differential.

*This position is eligible for a Commitment Bonus equivalent to 3 months salary with an agreement to stay with Polk County Behavioral Health for a period of at least 24 months.

• Master's degree in Psychology, Social Work, Recreational Therapy, Music Therapy, Art Therapy or a behavioral science field; 
• AND Credentialing as a QMHP or licensed as a LMFT, LPC, LCSW or PsyD;
• OR any equivalent combination of training, education, and experience that provides the required skills and knowledge to perform the essential functions of the job.

• Master's degree in Psychology, Social Work, Recreational Therapy, Music Therapy, Art Therapy or a behavioral science field; 
• AND (2) years of experience in the behavioral health field post-graduation
• AND Credentialing as a QMHP or licensed as a LMFT, LPC, LCSW or PsyD;
• OR any equivalent combination of training, education, and experience that provides the required skills and knowledge to perform the essential functions of the job.
