Brief Description
**RECRUITING TO FILL TWO (2) TEMPORARY POSITIONS**
ABOUT THE JOB
Performs a variety of client and community resource connection. Includes driving clients and community members to appointments.
WORK LOCATION
One position will be stationed in Monmouth and the other will be located in Falls City
MINIMUM REQUIREMENTS
Graduation from High School or equivalent and one (1) year Case Management Experience with demonstrated experience in community outreach services, providing health information, advocacy, support and assistance to individuals and or families or equivalent combination of education, experience and training.
SPECIAL REQUIREMENTS
Completion of HMIS Database training within three (3) months of employment. Must possess and maintain a valid Oregon driver’s license and automobile insurance, and have access to and use of a personal automobile as a condition of employment.
