Poppys Therapeutic Corners Mental Health Case Manager is responsible for providing
comprehensive case management services to individuals with mental health, social, and
behavioral health needs. This role involves assessing clients' needs, monitoring and
maintaining progress of the individualized treatment plan, coordinating care, and
connecting clients to community resources and support services to promote their
well-being and recovery.
As responsibilities may vary, our mental health case managers
perform the following duties :
Assessment and Treatment Planning :
Conduct comprehensive non clinical assessments of clients' mental health,
social, and behavioral health needs, strengths, and goals.
Collaborate with clients and multidisciplinary teams to develop, and review
individualized treatment plans that address mental health symptoms,
social challenges, and behavioral health concerns.
Identify and prioritize goals related to accessing community resources,
social support, and mental health interventions.
Community Resource Referral and Coordination :
Research and maintain up-to-date knowledge of community resources,
including mental health resources outside of Poppys Therapeutic Corners
services, social service agencies, housing assistance, and vocational
rehabilitation services.
Connect clients with appropriate community resources and support
services to address their mental health, social, and behavioral health needs.
Coordinate referrals and appointments with external agencies and service
providers, ensuring seamless access to resources.
Advocacy and Support :
Advocate for clients' rights and needs within the community, including
assisting with applications for benefits, housing, and other entitlement
programs.
Provide ongoing support and guidance to clients in navigating the complex
healthcare and social service systems.
Empower clients to self-advocate and build resilience in managing their
mental health, social, and behavioral challenges.
Crisis Intervention and Risk Management :
Provide crisis intervention and support to clients experiencing acute mental
health crises, social crises, or behavioral health emergencies.
Develop safety plans and collaborate with emergency services and crisis
intervention teams as needed.
Assess and manage risk factors related to suicide, self-harm, substance
abuse, or harm to others, ensuring appropriate interventions are
implemented.
Documentation and Reporting :
Maintain accurate and up-to-date client records, progress notes, and
documentation in compliance with agency policies and regulatory
requirements.
Self-Advocacy Skills :
Empower clients to develop self-advocacy skills and build resilience in
managing their mental health. Case managers encourage clients to identify
their strengths, set realistic goals, and take an active role in their recovery
journey.
