Job Description
POSITION SUMMARY :
The registered nurse is responsible for the delivery of patient care services through coordination, implementation, and supervision of the patients. The registered nurse follows the POC and supervises the staff delivering care. The registered nurse participates in quality improvement activities within the Agency promoting overall compliance with State and Federal guidelines and professional standards of practice.
DUTIES AND RESPONSIBILITIES :
Demonstrates leadership and provides direction to members of the patient care team through case management of patient home health episode of health care. Case management of patient services including :
- Intake and coordination of a referral form an acceptable referral source.
- lnitial in home evaluations and / or assessment.
- Establish the patients individualized treatment and medical record baseline.
- Collaborate with physicians, other agency staff : and contract services to coordinate and implement the POC.
- Supervises the implementation of the POC and the quality of care delivered to the patient.
- Provides in home supervision of Home Health Aide Services at least every 14 days for patient receiving aide services and documents the visit in the medical record.
- Provides supervision of LPN's assigned to patient caseload, through directing the implementation and evaluating the effectiveness of the nursing care and patient's plan of treatment.
- Re-evaluating the patient plan of treatment at least every 60 days, and when there is a change in the patient condition, and after a hospital stay, and a time of discharge revising the Plan of Treatment as needed to achieve patient goals.
- Documents direct patient services, coordination and collaboration with physician and other disciplines or provides in the care of the patient, ensuring document is complete, and complies with acceptable home health standards and Agency policy.
- Facilitates active and effective communication with team members as demonstrated through leadership of patient case conferences staff education or in services, and timely clinical decisions which provide guidance in the delivery of patient care.
- Documentation is timely and meets professional standards of practice, establishes compliance with state regulations and Medicare conditions of participation.
- Reviews caseload charts documentation at least eve,y 60 days to ensure compliance assess patient progress and effectiveness of care delive1y in meeting patient goals.
- Provides patients and staff supervision 24 hours a day when assigned on-call.
- Collaborating with leadership / management team and office staff to facilitate timely office processing of paperwork and
personnel information.

PHYSICAL QUALIFICATIONS :
Prolonged or considerable walking or standing. Able to lift, position, or transfer clients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.
Requirements
Minimum of 18 years of age with a high school diploma or G.E.D. Valid driving license and must be insurable by the agency insurance carrier. Specific vision abilities required by this job include distance vision, peripheral vision, depth perception and ability to adjust focus if required to drive a company vehicle. Ability to meet training requirements in accordance with Federal, State, and local standards, as well as organizational policies and procedures per ARSD guidelines. Ability to complete and pass training in Medication Certification to become properly authorized and certified to supervise participants in taking medications as prescribed by the physician. Able to read, write, and speak the English language in an understandable manner. Ability to solve practical problems and deal with a variety of situations. Ability to interpret a variety of instructions furnished in written, oral, or schedule form. Ability to assist participants served with transferring utilizing lifts, mobility reinforcement (one and / or two staff assist) utilizing gait belt. Ability to maintain a flexible schedule, which may include working weekends and holidays. Capable of working with developmentally disabled participants by communicating verbally or in other manners to instruct, demonstrate, train, and work cooperatively. Must pass a background check that meets policy. Must pass a post-offer drug test with negative results Ability to lift 50 to 60 lbs. and stand / walk for 75% of their shift. Ability to use vision at 20 in. or less and 20 ft. or more. Depth perception that allows for judging distance and space. Must be able to perform lifting, moving, flexing, bending, pushing, pulling, crouching, and kneeling, squatting, reaching above shoulders, and use of hand / wrist to support weight occasionally. Ability to complete gross and fine motor activities with hands and fingers frequently. Pushing wheelchairs and able to assist participants with mobility. Ability to use gait belts and lift apparatus safely, and able to train DSP for proper lift and gait belt use. Ability to properly address possible exposure to bodily fluids. Ability to handle possible exposure to profanity, violent, and / or offensive behavior from participants.
Salary : $28 - $30
