Porscha's Heart to Heart Homecare is looking to hire a dependable, compassionate and professional LPN case manager. Some job duties are taking vital signs, coordinating and providing care that is safe, timely, effective and client centered. Handing Home care case assignments, reviewing service plans, review case progress and supervision, overseeing caregivers, completing agency documentation, understanding and explaining client documentation. Must have a active and current license.
