Do you have a passion for nutrition and want to help others in Portage County?  The Portage County Health & Human Services department is hiring a part-time, 20 hrs / wk, Community Nutrition Educator!

The pay range starts at $28.72 / hr


What are the primary job duties?

 

Public Health Sciences

  • Facilitates implementation of community-focused initiatives to promote nutrition, breastfeeding and prevent disease, in settings including, but not limited to all Division of Public Health designated homes, community organizations and worksites, schools, and the community at large.
  • Performs comprehensive individual and family assessments which include health history, physical/blood assessment, growth monitoring, developmental assessment, psycho-social assessment, assessment of family functioning, assessment for substance abuse or domestic violence issues, and assessment of basic needs including food, housing, income, resources and supports, and access to health care.
  • Provides care coordination, referral and follow-up to individuals and families who are members of a vulnerable population and/or high-risk groups. Evaluates outcomes, effectiveness of plan, and makes changes as necessary.
  • Assess and provides nutrition education and counseling to individuals, families and community groups that are adapted to their unique needs, lifestyle, cultural and socioeconomic situation.

Community Dimensions of Practice

  • Establishes, expands, facilitates, and sustains initiatives, programs, and/or partnerships that engage key stakeholders and community members in order to address the community health priorities of nutrition, physical activity and breastfeeding.

Analysis and Assessment

  • Participates in Portage County Division of Public Health’s community health assessment and health improvement planning and intervention activities.
  • Develops and/or uses databases and software programs to gather and maintain needed data for nutrition, breastfeeding, and disease prevention, evaluation and quality improvement efforts.

Leadership and Systems Thinking

  • Demonstrates knowledge regarding, and compliant with, applicable professional practice guidelines and any other Federal and State laws and regulations applicable to practice as a public health professional.
  • Adheres to Occupational Safety and Health Administration standards, such as those concerning exposure to bloodborne pathogens, toxic substances, airborne pathogens or exposure to other hazards during routine assignments or assignments during public health emergencies.
  • Responsible for using quality improvement (QI) and performance management (PM) processes and/or techniques to improve the effectiveness of the respective public health program. This includes but is not limited to creating. Implementing, and evaluating performance standards and identifying, implementing, and assessing program quality improvement processes.

Policy Development and Program Planning

  • Assist in developing and implementing a community involvement process that assures the informational exchange necessary to provide Public Health’s input into appropriate plans and policies; identify potential barriers to new policy development and implementation.

Communication and Cultural Competency

  • Delivers targeted, culturally appropriate information to help individuals and groups understand local environmental public health policies, regulations, and code, utilizing appropriate methods for interacting effectively and professionally with persons of all ages and from diverse cultural, socioeconomic, educational, racial, and ethnic backgrounds, sexual orientations, lifestyles, and physical abilities.

Financial Planning and Management

  • Contributes to the development and preparation of proposals for funding from external resources for health promotion and disease prevention initiatives and programs.
  • Collaborates in activities such as time studies that contribute to the development of budgets and the efficient use of resources.

What are the minimum qualifications?
 
  • Education:
    • Bachelor’s degree in dietetics or food and nutrition OR
    • Master’s degree in nutrition or public health nutrition OR
    • Nutrition Dietetic Technician, registered (NDTRs) who is registered with the Commission on Dietetic Registration (CDR)
  • Registered Dietitian (RD) or Registered Dietitian/Nutritionist (RDN) who is registered under the (CDR) and is also a Certified Dietitian in the State of Wisconsin, or can pass the RD or RDN exam within 6 months of hire preferred.
  • WIC experience preferred.
  • Public health experience desired.
  • Breastfeeding training desired.
  • Bilingual in Spanish or Hmong desired.
  • National Incident Management System (NIMS)/Incident Command System (ICS) 100, 200 and NIMS 700, 800 certifications within twelve (12) months of employment.
