Porter is hiring in Galesburg, Illinois and is looking for Nurse Practitioners like you!
What You'll Do
- Conduct telehealth and in-home health risk assessments for members in your geographic area (you are required to conduct all types of visits)
- Evaluate past medical history, review systems, reconcile medications, and perform vital signs and physical exams
- Use evidence-based practice screening, including point-of-care testing as needed
- Identify diagnoses for care management by Porter Care Guides
- Actively manage medical treatment based on assessment findings
- Formulate lists of current and past medical conditions using clinical knowledge and judgment
What You'll Need
- Valid unencumbered Illinois NP license
- 3 years of patient care experience required (experience in primary care/adult/geriatric, and EMR skills preferred)
- A strong sense of empathy, compassion, and friendliness
- Exceptional communication skills, both written and oral, with the ability to assist others (patients and coworkers alike) with sincerity, respect, and compassion
