Positive Growth is seeking a Case Manager/Paraprofessional to join our Breakthrough program. This program serves individuals aged 16-30 who have experienced their first episode of psychosis within the past two years. This role involves working closely with clients, their families, and the multidisciplinary team. 

The (EIPP) Case Manager provides individualized supportive case management to agency clients with the goals of 1) assisting clients to achieve or maintain stability and independence and 2) providing assistance accessing resources to help clients meet their housing and other needs. 

Essential Duties:


● The case manager plays a crucial role in coordinating access to necessary services for
clients. This involves working closely with families, healthcare providers, community
resources/agencies, and other support services to ensure clients receive the appropriate
care and treatment.
● Responsible for assessing service needs. Must be familiar with the bio-psycho-social
assessments to understand the individual's condition to provide the appropriate case
management support required.
● Providing case management services; using a phase-specific recovery model, the case
manager provides case management services to clients and their caregivers. This includes
developing and implementing a care plan, monitoring progress, and making necessary
adjustments to support the client's recovery.
● Providing support to clients and their caregivers throughout the treatment process. This
may involve providing information, education, and emotional support to help them cope
with the challenges of psychosis.
● Complete timely documentation and meet minimum weekly service hours based on EIPP
requirements.
● Provide psychoeducation to clients and their families about psychosis, its symptoms,
causes, and available treatment options.
● Offer guidance on coping strategies, medication management, and stress reduction techniques.
● Promote family involvement and support families in understanding and navigating the
challenges of psychosis.
● Be available to respond to crises or urgent situations, supporting clients and families
during times of heightened distress.

Qualifications:

● Bachelor's degree in social work, psychology, counseling, or a related field, and work
experience with adults with severe and persistent mental illness and substance use
disorders or any combination of education, experience, and training which provides the
following knowledge, skills, and abilities:
● At least two years of related experience and/or training in mental health is preferred.
● Experience working with individuals experiencing psychosis or severe mental illness,
preferably in an early intervention setting.
● Strong interpersonal, communication, and collaboration skills.
● Knowledge of evidence-based practices for early intervention and psychosis treatment.
● Ability to work effectively within a multidisciplinary team and the demonstrated ability
to work with flexibility and efficiency in a fast-paced environment.
● Self-starter who is detail-oriented, well-organized, and enjoys working in a collaborative
environment.
● Computer proficiency is required to use Electronic Health Records. Valid OR driver's
license and excellent driving record required. Must complete state-required
fingerprint/background and DMV checks.
