Duties and Responsibilities
- Develop a housing procurement, financial and self- sufficiency case management plan with clients seeking assistance in the HOPWA HINT Housing Program.
- Assist in the development of and encourage adherence to a personal budget and budget counseling sessions; provide budget counseling and education to assist clients in establishing payment plans for past due bills and debts.
- Ensures that program deliverables are met or exceeded during the project period.
- Ensures that client records are maintained in compliance with Federal, State, and other grant requirements.
- Work in interdisciplinary teams to provide wrap-around services to program participants.
- Assist participants in the development of a strength-based/solution-focused individualized housing goal and action plan that promotes self-sufficiency.
- Work closely with community agencies and services to develop partnerships that will offer life skill training (e.g. employment training, financial literacy, job placement) for program clients.
- Acts as a patient advocate and liaison between the client and community service agencies.
- Assess housing-related requests efficiently.
- Ensures that program deliverables are met or exceeded during the project period.
- Ensure that client records are maintained in compliance with Federal and other grant requirements.
- Ability to translate federal housing regulations for client and landlord comprehension.
- Assist with completing project related reports and audits as needed.
- Document all client encounters appropriately and in a timely manner into HMIS and/or EMR systems.
- Attend regular PIHC staff meetings, trainings, and other meetings as requested.
- Attends and participates in regular HUD, HOPWA and associated conference calls for the HOPWA HINT program as needed.
- Attend project-related training sessions, meetings and conferences as assigned.
- Attends weekly team meetings.
- Complies with PIHC's policies and procedures.
- Learns the use of any data collection systems required by HRSA, HUD and/or HOPWA and can train project staff to utilize all required data collection systems.
- Participates in quality assurance reviews as assigned.
- Other duties as assigned.
Requirements
Minimum Qualifications
- Bachelor’s degree in social service specialty (e.g. social work, psychology, sociology, public health).
- Experience in housing, social work, or case management is highly preferred.
