We are a nephrologist-led renal care company helping patients in post-acute settings recover and return home after their hospital stay. We operate primarily in skilled nursing facilities (SNFs), working as an extension of the in-house nursing team and in collaboration with other consultants including cardiology and pulmonology. We aim to offer our partner homes a team-based, renal-focused layer of clinical oversight to ensure patients do not experience preventable renal complications. The Role :

We are seeking a full-time nurse practitioner or physician assistant in the Chicagoland area to visit our patients in SNFs. The role will entail visiting ~20-25 patients a day, typically across several nursing homes. You will work in close consultation with our nephrologist leadership to :

  • Conduct physical exams, check vitals, etc.
  • Review labs (H&Ps, BMPs, CBCs, etc.), monitor changes and interpret results
  • Consult with other providers, especially for comorbid patients
  • Manage medications (drug interactions, reduce excessive use, etc.)
  • Manage nutrition (fluid / electrolyte balance, etc.)
  • Routinely review patient panels to flag those requiring renal care
  • Attend interdisciplinary meetings with SNF nursing team
  • Train home’s nursing staff on renal care basics and when to escalate issues
  • Answer questions and provide support to patients and their families
  • Document visits into EMRs
  • Manage overall care to prevent Acute Kidney Injury (AKI) and other renal issues

Since we are not proceduralists, our practice is limited to consultative work – visiting patients, communicating with providers, managing medications, reviewing labs, etc. Most often our impact is driven by making recommendations to the primary service. Ideal candidates will have experience in post-acute settings, with geriatric patients and renal care.

Requirements :

  • NP / PA / APRN certification
  • Licensed to practice in Illinois
  • DEA license
  • Vehicle transportation
  • Compensation & Benefits :

  • Expected salary range as a typical, full-time practitioner $130-140k annually
  • Ability to scale up compensation from typical range, based on productivity
  • 1099 contract-to-hire arrangement, eligible to opt into hire after ~90 days

    Salary : $130,000 - $140,000
