We are a nephrologist-led renal care company helping patients in post-acute settings recover and return home after their hospital stay. We operate primarily in skilled nursing facilities (SNFs), working as an extension of the in-house nursing team and in collaboration with other consultants including cardiology and pulmonology. The Role :
We are seeking a full-time nurse practitioner or physician assistant in the Chicagoland area to visit our patients in SNFs. The role will entail visiting ~20-25 patients a day, typically across several nursing homes. You will work in close consultation with our nephrologist leadership to :
- Conduct physical exams, check vitals, etc.
- Review labs (H&Ps, BMPs, CBCs, etc.), monitor changes and interpret results
- Consult with other providers, especially for comorbid patients
- Manage medications (drug interactions, reduce excessive use, etc.)
- Manage nutrition (fluid / electrolyte balance, etc.)
- Routinely review patient panels to flag those requiring renal care
- Attend interdisciplinary meetings with SNF nursing team
- Train home’s nursing staff on renal care basics and when to escalate issues
- Answer questions and provide support to patients and their families
- Document visits into EMRs
- Manage overall care to prevent Acute Kidney Injury (AKI) and other renal issues
Since we are not proceduralists, our practice is limited to consultative work – visiting patients, communicating with providers, managing medications, reviewing labs, etc. Most often our impact is driven by making recommendations to the primary service.
