As part of the ACT you must be willing to go out into the community to provide services. This position requires the desire to work as part of a team.ESSENTIAL FUNCTIONS :
- Participate in morning meetings and staff meetings.
- Evaluate need for psychotropic medication; prescribes medications as needed, educates clients and relatives about these medications and their effects, reviews and adjusts dose level as needed, and documents all medications in consumer's electronic chart.
- Conduct visits in the community in accordance with NYS Office of Mental Health (OMH) licensing standards.
- Coordinate consumer access to medications with the RN and LPN nurses including administering injectable medications when needed.
- Order routine blood work and / or toxicology testing as needed.QualificationsEDUCATION AND EXPERIENCE :
- License in NYS as a Psychiatric Nurse Practitioner
- Minimum of three (3) years working with people with serious mental health conditions, co-occurring substance use disorders, and homelessness.
Salary : $160,000 - $175,000
