JOB SCOPE: The CCBHC Psychiatric Rehab Worker will provide direct care individual and groups services to assist individuals to overcome mental health barriers that may have interfered with their ability to function independently in the community. The intent of the psychiatric rehabilitation services is to assist children and adults to fully restore their functional level. The provision of services will focus on skill building in areas of socialization, education, and employment. This position operates as part of a multi-disciplinary CCBHC clinical team and requires collaboration with the clinical treatment team, as well as external entities. CCBHC Psychiatric Rehabilitation Worker is responsible for participating in mobile crisis team, with rotating on-call coverage to respond to community-based crises outside of clinic operating hours.
ESSENTIAL FUNCTIONS:
- Assist with identifying meaningful life goals and objectives.
- Assists individuals improve self-management to reduce psychiatric symptoms or relapse of substance use that interfere with their daily living.
- Provide psychoeducation, coping skill training, stress management, anger management, travel training, dietary and wellness education, financial management, and similar services to improve life skills.
- Assist individuals in developing and pursuing leisure and recreational interests in their community.
- Assist with securing employment, including assistance in pursuing, securing, and retaining paid employment and sustaining that employment.
- Responsible for emergency on call after hours coverage for the Crisis Team.
- Performs other duties, consistent with the goals and objectives of the program, as directed by CCBHC or PCMH Administration.
Schedule Shift: Monday-Friday 9AM to 5PM
Salary: $55,000 yearly
Qualifications
SKILLS AND KNOWLEDGE:
- Experience working with mental health and substance use disorder populations.
- Knowledge and experience of electronic health records (specifically AWARDS by Footholds) preferred.
- Must be able to use outlook, word, excel.
- Must be reliable and able to work independently and understand the importance of maintaining confidentiality.
EDUCATION AND EXPERIENCE:
- Bachelor’s degree in the Human Service field.
- Experience in behavioral health, substance use or other social service settings.
- Experience with facilitating groups.
- Ability to work collaboratively as part of a team.
- Must be outcome oriented in a fast-paced environment with deadlines.
Salary : $55,000
