Position Overview

We are seeking dedicated and skilled Registered Nurses (RNs) to join our reputable Certified Home Health Agency (CHHA) team. In this full-time role, you will provide high-quality, comprehensive nursing care to patients in the comfort of their homes. Your expertise will help manage and coordinate cost-effective healthcare services aligned with nursing standards, contributing to the well-being of our clients. This position is ideal for local candidates who reside in or near New York City.

Key Responsibilities

• Deliver direct patient care following our established policies, objectives, and nursing standards.
• Provide hands-on care, including wound management, IV and catheter handling, and medication administration.
• Engage in case management tasks, encompassing scheduling, assessments, and documentation.
• Evaluate patients' physical, psychological, and social needs to formulate effective care plans.
• Travel to patients’ residences for initial assessments and follow-up visits.
• Collaborate with a multidisciplinary team, reporting to regional leadership.
• Participate in team activities, including mentorship and performance feedback.
• Accurately document all services rendered as per governmental regulations and organizational policies.
  
  

Required Skills

• Valid Registered Nurse (RN) license/certification in New York (NY).
• Strong clinical experience, particularly in Medical-Surgical or Home Health.
• Excellent communication and interpersonal skills.
• Proficient in case management and patient documentation.
• Ability to assess and respond effectively to patients' needs.
  
  

Qualifications

• Minimum of 2 years of nursing experience in relevant healthcare settings.
• A Bachelor’s degree is preferred but not mandatory.

Compensation And Benefits

• Base Salary: $97,100.
• Additional compensation based on clinical experience and education:
• $750 per year of clinical experience
• $1,700 for a Bachelor’s degree
• $1,950 for a Master’s degree
• $1,300 for nursing certifications (e.g., wound care, pediatrics, psychiatrics)
• Eligible for annual raises of 3-4%.
