Key Responsibilities

• Deliver nursing care consistent with our organization’s philosophy, goals, and nursing standards.
• Provide direct, hands-on patient care, which includes wound management, medication administration, and IV/catheter oversight.
• Manage case responsibilities including patient assessments, documentation, scheduling, and referrals.
• Evaluate the physical, psychological, and social needs of patients to create effective care plans.
• Travel to patients’ homes for care delivery and follow-up visits.
• Collaborate with regional leadership and participate in team activities, enhancing the orientation of new team members by providing constructive feedback.
• Maintain thorough documentation of patient services in compliance with government regulations and organizational policies.
  
  

Required Skills

• Active Registered Nurse (RN) license in New York.
• A minimum of 2 years of experience in medical-surgical settings or home health.
• Strong clinical assessment and case management skills.
• Excellent communication abilities and teamwork orientation.

• Bachelor’s degree in nursing is preferred, but not mandatory.

Compensation And Benefits

• Base salary of $97,100 per year.
• Additional compensation of $750 for each year of relevant clinical experience.
• Bonuses for educational qualifications: \(1,700 for a Bachelor's degree, \)1,950 for a Master's degree, and $1,300 for specific nursing certifications.
• Eligible for annual raises of 3-4% based on performance.
