What are the responsibilities and job description for the Service Coordinator position at Potomac Center, Inc.?
- Coordinate all necessary services for individuals;
- Complete all necessary paperwork regarding the needs of the individuals;
- Responsible for documentation of all meetings and contacts;
- Support and assist the individuals in developing skills that will further their independence, increase their community involvement, and improve their quality of life;
- Monitor the health and medical needs of the individuals;
- Adhere to all Potomac Center policies and procedures in regards to managing individuals and miscellaneous house funds;
- Display, mentor, and role-model professional conduct, socially accepted behavior, a positive attitude, and team building at all times;
- Facilitate Data Reviews/ IHP meetings and ensure that all components of the individual’s IHP are accurately implemented by team members;
- Implement and train specific individual behavioral intervention plans;
- Ensure that all appropriate paperwork and documentation is completed;
- Maintain an active and open program with the individuals’ family and any relevant outside agency;
- Schedule, coordinate, and document the Individual Habilitation Plan meetings;
- Develop a written treatment plan for each individual in assigned home in conjunction with appropriate team members;
- Responsible for notifying guardians of any critical incidents regarding the individual, such as attempted suicide, changes in behavior, etc, concerning care;
- Facilitate and participate in meetings to discuss individual’s progress, difficulties, etc.;
- Complete ICAP, ABAS and GARS as necessary;
- Effectively communicates as evidenced by conveying necessary information accurately and asking questions when clarification is needed;
- Maintain confidentiality of individual, personnel, and Potomac Center information;
- Assist in providing coverage for medical and counseling appointments;
- Report or effectively communicate all information pertinent to the continuity of care of the individuals to team members on a routine basis;
- Recognize situations which must be reported to Child or Adult Protective Services and inform appropriate services of the same or ensure such reporting has occurred;
- Identify and define problems, collect data, establish facts, and draw rational conclusions with the ability to be detail oriented;
- Possess the ability to communicate in a manner which demonstrates respect for the dignity and rights of all individuals, supervisees, and other team members;
- Establish and maintain a positive relationship with state workers, biological families, guardians, and adults as appropriate;
- Participate in CQI activities as designated by the Program Manager or his/her superiors;
- Serve as an emergency resource person on an as needed basis;
- Maintain professionalism at all times and practice appropriate work ethics;
- Attend all required training as outlined in policies and procedures such as, but not limited to, First Aid, CPR, Safety-Care Intervention Strategies, etc.;
- Perform other related duties incidental to the work described herein as assigned by the Program Manager or his/her superiors;
- Possess valid driver’s license.
