Position Duties and Responsibilities:

  • Perform a full range of pharmacy technician procedures. Perform pharmaceutical dispensing duties including filling new outpatient prescriptions, refilling prescriptions, and entering orders into databases. Perform information consultation duties including supporting new and refill prescriptions, supporting patient requests, supporting physician’s requests, monitoring for drug interactions, and reporting adverse drug reactions. Perform quality improvement duties including performing drug storage inspection, reviewing expired supplies, and producing error and workload reports and documentation. Perform supply process duties including placing new orders, stocking/restocking shelves, inventory maintenance, producing not in stock reports. 
  • Possess a working knowledge of pharmaceutical terminology, common drugs, medication dosage forms, storage and security requirements for drugs, and the operation of common pharmacy equipment. Process extensive knowledge of pharmaceuticals to include therapeutics, dosage, nomenclature, side effects, interactions, and incompatibilities. Must have the capacity to be intimately familiar with all aspects of the Outpatient and Inpatient Drug Distribution System. 
  • Understand the process on outpatient prescriptions or inpatient medication orders. Assist pharmacist in selecting and preparing medications. Maintain necessary prescription files and other records in accordance with applicable regulations and standard operating procedures. 
  • Maintain thorough familiarity with MTF therapeutic formulary, dispensing policies, and requirements for eligibility for care. Display a helpful, courteous manner to patients, explaining options, and recommending solutions based on facts. Provide drug information to other health care professionals and patients when requested. Shall utilize the applicable electronic medical record system for task. The Government will provide training in the use of these systems implemented by the U.S. Army. Have sufficient skills to complete all required paperwork, utilize paraprofessionals as appropriate and perform documentation and patient care in accordance with TJC guidelines. 
  • Assume additional administrative or clinical duties as requested or needed to ensure sustainment of normal clinic operations and ensure compliance with Joint Commission (JC) standards, to include, but not limited to, ensuring enforcement of clinic Standard Operating Procedures (SOPs) and standards, entering medication errors and Adverse drug events, conducting monthly peer reviews, and attending staff meetings. Knowledgeable of Quality Improvement responsibilities which include upholding established patient care and practice standards, review and submit reports a required, participate in all mandatory training. 
  • Direct Patient Care: Contacts patients via phone for reviews medical record, assesses risk and need for clinical pharmacy visit, documents in electronic health record. Collaborates with Health Care Provider (HCP) for further actions and manages clinical pharmacist appointment schedule for appointments, appointment reminder and managing cancellations/no-shows. 
  • Indirect Patient Care: Assists the clinical pharmacist in data collection for various Pharmacy committees such as Pharmacy and Therapeutics (P&T), Medication Management Committee (MMC), Medication Use Evaluation (MUE), Antibiotic Stewardship Committee, and Sole Prescriber (SP). Helps with drug information inquires involving drug formulary, available dosage forms, availability of medication. Prepares educational materials for patients and HCPs. 
  • Participate in population health initiatives and processes to improve patient safety by assisting in acquiring/managing patient lists that may benefit from clinical pharmacy services. Collaborate with team members to optimize MEDCOM’s performance metrics for clinical pharmacy and the Army Medical Home (AMH) team’s performance metrics such polypharmacy, HEDIS and other performance improvement programs. Can perform medication distribution functions of outpatient prescriptions if required. 
  • Supports the Polypharmacy Medication Analysis & Report Tool (POLY-MART) reports for beneficiaries and active duty. Assist the AMH/CBMH, Warrior Transition Unit (WTU), and other specialized clinical pharmacy clinics (lipid, anticoagulation, etc.). Manages clinical pharmacy consults to prioritize, schedule and documents responses. Provide updates and maintains polypharmacy/patient tracking log as well as manages Sole Provider patient list communications with Express Scripts (ESI). Documents patient encounters by typically generating Telephone Consults [TCONS]), in the electronic health record (EHR). Utilizes Tri-Service Workflow (TSWF) Alternate Input Method (AIM) tools to support documentation and forwards to the clinical pharmacist for review/signature as appropriate. 

 

Minimum Qualifications: 

  • Graduate from a formal pharmacy technician program accredited by the American Society of Health-System Pharmacists (ASHP) or a formal pharmacy technician program (i.e., technical, hospital, or retail-based program) or a formal medical services training program of the United States Military. 
  • Have a minimum of three (3) year experience within the last 5 years as a Pharmacy Technician. 
  • Certified by the Pharmacy Technician Certification Board (PTCB). 
  • Must be a US Citizen.
