What are the responsibilities and job description for the Resident Care Coordinator position at Powell Valley Assisted Living and Memory Care?
General Description of the position: Provide administrative support, conduct Resident assessments outside of the Community, review and prepare Physician’s Orders, review MARS for survey compliance and function as Med-Aide when needed. Essential functions:
Assess the health condition and health care needs of the resident before admission. Serve as liaison between facility, applicant’s family and appropriate medical personnel. (Assist facility to problem solve when change in resident’s health status occurs.) Coordinate with other community health care professionals, such as the home health agency, hospice, physician’s offices, etc. (to acquire use of their expertise when necessary.) Actively participate in the development of resident service plan. (Participate in service plan conferences, as needed. Establish a medication & treatment administration program before admission. (Appropriate medication documentation and administration system for each resident. Monitor all medications and prescriptions. Extraordinary tasks: Finger sticks for diabetics. Peri-care on females (vaginal secretions). Handling soiled linen. Clean up of potentially contaminated fluids. Attending to an injury where blood is present. I understand that because of the above-mentioned routine tasks, I could be at potential risk for contracting a bloodborne pathogen, including but not limited to: non-A hepatitis, non-B hepatitis, hepatitis B (HBV), delta hepatitis, syphilis, malaria and/or human immunodeficiency virus (HIV). Therefore, the company will provide me with the appropriate training in Universal Precautions and OSHA ‘s Exposure Control Standards in order to minimize any possible exposure. They will further provide me with an HBV vaccination if I so choose. It will be my responsibility to adhere to all Exposure Control Policies as set forth by the company I have read the above and have received training on Bloodborne Pathogens. Nile Living Revised 1.1.21 Working conditions: Lifting up to 200 ponds Driving up to 4 hours daily Knowledge, skills and abilities required: Must have excellent communication skills both written and verbal, with Residents, their families, staff and Management Company and/or Owners Equipment used: Office equipment including but not limited to: telephones, computers, copiers and printers Occasional tasks may require the use of (but not limited to) lifts, needles and driving company vehicles. Qualifications: A genuine concern for the elderly. Must perform excellent customer service skills Ability to lead and motivate people. Capable of making mature judgments. Knowledge of basic scientific principals necessary to provide competent care. Ability to assess human responses to actual or potential health problems. Understanding the normal dosage, action and common side effects of most medications in common use. Remain drug free. Well groomed. Telephone answering skills. Interpersonal relationship skills. Oral and written communication skills. Ability to manage and prioritize a large work load. Ability to maintain confidential information. Dependable. Must have First Aid Training Certificate. Must have abdominal thrust training. Education: Associates Degree in a field related.
