Description
Major Responsibilities As Activities Coordinator (but not limited to):
- Records in the residents’ medical records necessary documents: activities interest, needs assessment, activities care plan, daily participation and subsequent reassessment
- Organizes a program of both group and individual activities based on the resident’s individualized needs as identified in each residents care plan assessments
- Maintains a balanced recreational activity program, including physical, social, religious, intellectual and emotional functions
- Ensures that group activities are scheduled at times that will provide for maximum involvement by interested residents. This will include weekends and evenings
- Communicates residents’ programs to appropriate parties including residents, residents’ families, volunteers, and facility staff.
- Posting large monthly calendar(s) in centralized location(s)
- Posting small calendars in residents’ rooms
- Send family members a monthly calendar per family wishes
- Responsible for the transportation of residents within the facility from one area to the designated area where activities occur
- A member of the facility’s care plan team
- Knowledgeable, able to provide in-services related to activity programs
- Effectively manages the Activity Department budget on a monthly basis
- Supervises volunteers in the Activity Department
- And other duties as assigned.
Qualifications
Education, Training, and Experience:
Two (2) years of experience, in an activity department of a long term care facility, preferred.
- References indicating honesty, good attendance, dependability, performance and leadership abilities.
- English language skills adequate for effective communications with staff, patients, medical and allied support personnel.
- Able to read and follow directions with adequate visual and hearing acuity.
- Positive supportive approach to position and co-workers
- Computer skills
- Good general health
- Ability to deal positively in stressful situations
- Willing to work varied shifts and weekends/holidays when appropriate
Certificates, Licenses, and Registrations:
- BLS certification
