LPN Long Term Care (Full-time)
Description
Adhere to District Policies.
- Observe and enforce safety precautions for all District personnel, patients / residents and visitors
- Control resources and processes toward the attainment of optimum provision of service
- Provide care utilizing current nursing skills and interventions
- Collect data regarding the patient needs, environment, his / her family and early identification of discharge and educational requirements coordinating this information with appropriate department / agencies to assure continuity of care from admission to discharge
- Promote interdepartmental cooperation
- Practice within the scope of education, demonstrated competence, and nursing experience
Major Responsibilities : (but not limited to)
Collect and analyze pertinent objective and subjective data regarding the health status of the patients / residentsPlan and implement nursing care which will assist the patients / residents to maintain or return to a state of health or supportive dignified deathCommunicate significant changes in the patient / resident status to appropriate members of the health care team in a time period consistent with the patient / resident need for careDocument, on essential patients / residents’ records, the nursing care given and the patients / residents response to that careServe as patients / residents advocateBe accountable for the safety of patients / residents receiving service by :a) Delegating selected nursing functions to others in accordance with their education, credentials, and demonstrated competence
b) Supervising others to whom she / he has delegated nursing functions
c) Maintaining knowledge and understanding of the laws and rules regulating nursing and function within the legal scope of the Licensed Practical Nurse Practice Act.
d) Being responsible and accountable for his / her nursing acts based on and limited to the scope of her / his education, demonstrated competence and nursing experience
e) Obtaining instruction, supervision, and consultation as necessary before implementing new or unfamiliar techniques or practices
f) Maintaining current knowledge in his / her field of practice
g) Respecting the patients / residents right to privacy by protecting confidential information
Reporting unsafe nursing, medical acts and practices, and illegal acts as define in Idaho codesPerform acts that require knowledge, judgment and skill based on the principles of biological, physiological, behavioral and sociological sciences grounded in the elements of the nursing process which includes the observation assessments, care, and health teaching in the maintenance of health or prevention of illnessEvaluate and update the Nursing Care Plan to reflect the current needs of the patient / residentIdentify potential discharge needs during the time of care and relay this information to the appropriate resourcesDocument nursing interventions, patients / resident’s response to the intervention and disposition of licensed providers’ orders, noting correct time, date, and signature, report any unusual occurrenceMaintain a clean, safe environment for patients / residents, personnel, and visitorsEffectively communicate with District personnel, patients / residents, visitors and general publicAdherence to O.S.H.A. Blood borne Pathogens Exposure Control Plan, Universal Precautions, District fire, disaster, hazardous material, MDS plan, and safety plan, use proper body mechanicsAble to function in flexible staffing situations as required by patient resident census and acuityResponsible for all information from nursing staff meetings, in-services, mandatory meetings and District communicationsPerform other related duties as may be requested or assignedUtilizes educational opportunities available and seeks self-improvementBLS certifiedMedication certified, IV certifiedQualifications
Graduate of an accredited nursing program
State of Idaho licensedOne or more years experience in acute / LTC careReferences indicating honesty, good attendance, dependability, performance and leadership abilities.English language skills adequate for effective communications with staff, patients, and publicPositive supportive / coaching approach to co-workers.Computer skillsGood general healthWork is active, some lifting and twisting requiredAble to tolerate long periods of standing and walkingAbility to deal positively in stressful situationsWilling to work varied shifts and weekends as needed
