POSITION: Nurse Practitioner/Physician Assistant
DEPARTMENT: Family Medicine
REPORTS TO: Director of Department Operations
POSITION SUMMARY: Under the supervision and direction of the supervising physicians, the Nurse Practitioner/Physician Assistant provides direct patient care to (primarily) established patients of the practice. Care involves initial and subsequent histories and physical exams for both acute and routine office visits; appropriate documentation of all patient evaluations; involvement with initial triage and resolution of patient office calls; provision of emergency care to patients presenting to the office and/or in need of urgent attention throughout the clinic. The Nurse Practitioner/Physician Assistant will work collaboratively with the physicians in providing care to the patients in accordance with standards outlined by Indiana law. He or she will function within the legal boundaries of their advanced practice area and shall have and utilize knowledge of the statutes and rules governing state and federal drug laws and regulations, state and federal confidentiality laws and regulations, and state and federal medical records access laws.
ESSENTIAL FUNCTIONS AND JOB RESPONSIBILITIES-NP:
Education/Certification/License: Will need applicable licenses DEA & CSR prior to starting
NP - Must meets Indiana State Board of Nursing requirements for Nurse Practitioner licensure.
PA - Must graduate from an accredited educational program for physician assistants.
Knowledge, Skills, and Abilities:
Must have appropriate experience working with patients of all ages. Previous experience working with patients with acute and chronic care issues. Knowledge of medications and interactions. Ability to assess patients by using advanced nursing knowledge and skills to identify abnormal conditions, diagnose health problems, implement treatment plans, evaluate patient outcomes, and collaborate with physicians. Uses critical thinking skills to make independent decisions, commensurate with autonomy, authority and responsibility of a nurse practitioner. Mathematical ability to calculate correct dosages for patient medications. Possesses excellent organizational skills and communication skills. Ability to work independently, manage multiple priorities and meet deadlines. Excellent medical documentation skills, for both letter dictation and medical record. Computer skills necessary to participate in patient scheduling, dictation review, and diagnostic results reporting

ESSENTIAL FUNCTIONS AND JOB RESPONSIBILITIES-NP:
- Perform initial and subsequent history and physical examinations on patients presenting to the clinic for scheduled appointments; these may be reviewed and co-signed by the supervising physician.
- Appropriately document evaluations and progress notes for patients on initial and follow-up visits. To accumulate, correlate, and present the above data.
- To screen patients to determine need for further diagnostic or therapeutic care and follow-up.
- To refer patients to other medical specialists’ subject to the supervising physician’s discretion.
- To prescribe or provide diagnostic evaluations, subject to the supervising physician, including, but not limited to, laboratory, immunologic, radiographic, and imaging studies.
- To prescribe and dispense therapeutic agents, subject to review of the supervising physician.
- To perform office procedures under the direction and supervision of the physician.
- To provide education and counseling for the patient and the patient’s family in preventative care, medical conditions, and the use of prescribed treatments and drugs.
- Emergency Duties: The NP should initiate activities but also make reasonable attempt to summon a physician as soon as possible. Actions may include the treatment of anaphylaxis as well as CPR and advanced cardiac life support as detailed in the AHA guidelines.
- Misc. Duties: Administrative duties determined by supervising physician as well as by Clinic administration, education of health care student, and other duties as negotiated with the supervising physician.
- The NP will adhere to the guidelines of collaboration as agreed to with the supervising physician.
- The NP will maintain regular communication and planning with the supervising physician so that chart reviews meet or exceed the established SBC guidelines and are recorded for audit records.
- The NP will provide evidence of continuing medical education credits to remain current in the field and to meet SBC requirements.
- Performs comprehensive medical examinations on new and established patients to diagnose and treat medical and surgical conditions.
- Documents visits and procedures using standard E&M guidelines and CPT codes within established clinic timeframes and in acceptable format, which supports billing of all services.
- Submits completed fee tickets for services rendered, which is supported by medical record documentation.
- Assists physicians in the hospital and in the office setting with procedures, post op care, and follow up.
- Orders labs, radiology procedures, diagnostic testing and consults on patients as part of their treatment plan.
- Prescribes medications appropriate to patient diagnosis and within prescriptive authority of provider.
- Complies with the Physician Extender Policy by working collaboratively with the supervising physicians on chart review logs and didactic training sessions.
- Maintains CEU’s to comply with ISBH requirements for active licensure and to stay current in the field
- Meets MHSB and SJRMC medical staff requirements to maintain hospital privileges.
- Assists in the supervision of support staff to include training, lectures, demonstrations
