POSITIONSUMMARY:
Community Health is seeking a Family Practice Physician to work as part of our Primary Care Physician teamwithin the greater Rutland Region in the central Vermont area.Community Health has 5 Primary Care locations:Rutland, Pawlet (Mettowee), Castleton, Brandon, and Shoreham Vermont.Community Health has two UrgentCare locations within our Rutland and Castleton Primary Care clinics. Physician candidates are expected to beknowledgeableandskilledinthedeliveryofprimary care.
RESPONSIBILITIES (includes,butarenotlimitedto):
Community Health Center sites are certified Level 111 Medical Homes, with a highly developed and supportivepanel management and case management system in place. Community Health also has a robust integratedbehavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed ClinicalSocialWorkersembeddedwithinitsprimarycaresites.Wehaveafullyintegratedelectronicmedicalrecordandpractice management system with robust interfaces with our local hospital. After hours call (triage only) issharedamongsites.Callresponsibilityisminimal.

- MustbeeligibleforlicensingbytheStateof Vermont
- DoctorofMedicine(MD)andDoctorof OsteopathicMedicine(DO)accepted
