The Hospital at Home Program at UMass Memorial Health is currently seeking a full-time Hospitalist Physician. The successful candidate will be board-certified/eligible in Internal Medicine or Family Medicine. Consideration will be given to both experienced physicians and graduating residents/fellows.
Position Summary:
The Virtual Home Hospitalist is an integral member of the UMass Memorial Medical Center (UMMMC) Hospital at Home (HAH) clinical team, responsible for admitting, transferring, and caring for patients from UMMMC emergency departments and med/surge floors to the HAH program. After completing initial training at the Worcester command center, the Virtual Home Hospitalist can practice virtually within Massachusetts, provided they have a HIPAA-compliant private space, stable internet, and the ability to travel to the command center for training or urgent clinical issues.
This role involves collaborating with HAH nursing and paramedic teams to deliver telehealth-facilitated inpatient-level care at home. Responsibilities include providing clinical consultation for patient admissions, supporting care planning, responding to patient needs, and maintaining high standards of care for vulnerable populations.

Key Responsibilities:
- Direct Patient Care:Provide 12-hour virtual care shifts (7:00 AM - 7:00 PM), including emergency overnight coverage on 50% of active call nights (7:00 PM - 7:00 AM).
- Patient Admissions:Collaborate with the UMMMC patient acquisition team for new patient referrals and perform “admission tuck-in” video visits focusing on care planning and medication reconciliation.
- Supervision & Collaboration:Supervise and mentor residents and APPs, and work closely with nursing, paramedic, and APP teams to provide coordinated care.
- Clinical Flexibility:After initial training, work remotely in a HIPAA-compliant space with occasional in-person duties at the Worcester-based command center for meetings and educational events.
- Program Improvement & Participation:Contribute to continuous care process improvements and actively participate in regularly scheduled team meetings.
- Weekend & Holiday Coverage:Participate in clinical coverage for weekends (two per month) and holidays (two major and one minor per year).
- Additional Clinical Role:Opportunity to work as both a Virtual Home Hospitalist and a Brick-and-Mortar Hospitalist if service needs align.
License/Certification/Education:
Required:
- MD or DO degree
- MD or DO licensure in the State of Massachusetts
- BC/BE Internal Medicine or Family Medicine
Required:
- Recent inpatient level acute care experience
- Comfort with telehealth or growth mindset to learn telehealth
- Comfort with closely partnering with patient, family, unskilled and skilled caregivers in the home to create and execute care plans
- Comfort with care of the complex geriatric patient in low control environments
- Comfort with uncertainty and holistically balancing risks of brick/mortar hospitalization and home hospitalization to provide second-to-none-quality care
- Highly collaborative, passion for working with small, dedicate care team
- Ability and interest to pro-actively manage a panel of inpatient-level acutely ill patients using combination of remote- and in-home resources
